INDIAN RIVER COUNTY <br />Date: 8-21-85 <br />Work Authorization No. 1 for Consulting Services (Wastewater) <br />Project No. 1 , County Serial No. (Masteller & Moler <br />Associates, Inc.) <br />I. PROJECT DESCRIPTION <br />Title: Consulting Engineering services relative to modifications of <br />the Whispering Palms pumping station including removal of wet <br />well lid and superstructure; removal of pump, piping and valves; <br />removal of electrical controls/panel; extend existing wet well <br />to grade, construct new wet well cover; install new duplex pumps <br />and electrical controls/panel; install new valve chamber and <br />connect to existing force main. Budget estimate for construction <br />cost is $28,000.00. <br />II. SCOPE OF SERVICES AND LUMP SUM FEE FOR SERVICES <br />Reference is made to the "Agreement for Professional Services" <br />dated June 5, 1985 and specific Article numbers and paragraph <br />numbers which are listed hereinafter to describe the scope of <br />services included on this project: <br />A. Project Design Services per Article 4, paragraph 4a. and <br />4b. <br />B. General Services During Construction per Article 5, para- <br />graph 5a. and 5b. <br />C. Supplementary and Special Services per Article 6, paragraph <br />6c., 6d., 6e., 6f., and 6g. In addition, our services <br />shall include field surveys necessary for design and prep- <br />aration of construction plans and assistance to the County <br />in obtaining various permits and approvals for construction <br />(permit fees to be paid by the County). <br />D. Payment for Services per Article 8, paragraph 8a.(1) and <br />8a.(2), the fee for A, B, and C shall not exceed a total <br />of $3,800.00.
