Baseline: Certificate of Accreditation from <br /> the National Accreditation Commission (NAC), Program Quality Assessment (PQA). <br /> The center has maintained accreditation through a nationally recognized accrediting agency, <br /> National Accreditation Commission (NAC) since May 2111, 2015 First annual report is due May <br /> 21st, 2016. Program Quality Assessment(PQA) scores for the centers are 4.33/5.00. <br /> Cumulative Reports <br /> 1. 9 unduplicated infants and toddlers served this quarter; Year to Date 53 <br /> 2. 19 unduplicated children served this quarter; Year to Date 1 1 1 <br /> 3. 4 referrals made to other CSAC agencies Year to Date 20 *ELC children <br /> 4. 1 families/children were not served due to lack of funds; Year to Date 3 <br /> 'a Substance Awareness Center of Indian River County <br /> Re-Direct Program <br /> 2014-15 Contract— $100,000 <br /> Total Children Served: 166 <br /> Cost per Child $602.41 <br /> Program Description: Re-Direct targets adolescents ages 12-18 with substance use and co- <br /> occurring disorders. The Re-Direct program proposes to re-direct and motivate adolescents <br /> and their patents towards a substance free, pro-social lifestyle. The Center follows science- <br /> based best practices and modalities, which are designed to help engage and engage families <br /> in positive change: utilizing a non-judgmental, motivational and proven effective approach for <br /> successful outcomes. <br /> GOALS/OUTCOMES REPORT <br /> Outcome # 1: 80% of program participant will demonstrate reduced use of ATOD as <br /> evidenced by negative drug test results during the course of the program and as a requirement <br /> - 36 - <br /> 154 <br />
