Davis, P.E., <br />Public Works Director <br />SUBJECT: Indian River Boulevard South <br />E tension - Change Order #10 <br />REFERENCES: <br />DESCRIPTION AND CONDITIONS <br />Change Order No. 10 has been prepared to provide for the following changes <br />in the subject project: <br />1) Add two stormwater catch basins at the WalMart driveways along Indian <br />River Boulevard to correct severe erosion problems in this area. This <br />work will reduce future maintenance. <br />Cost = $6,360 <br />2) Construct at 10' x 3'6" conflict manhole at Old Dixie Highway and 12th <br />Street. It has been discovered that a Southern Bell Telephone line is <br />in conflict with the proposed stormdrain. Southern Bell has agreed to <br />fund $4,250 or 50% of this work. <br />Cost by Southern Bell $ 4,250 <br />Cost to County $ 4,250 <br />Total Change Order cost $14,860 <br />ALTERNATIVES AND ANALYSIS <br />The work included in Item 1 in the Change Order is desirable to reduce <br />future maintenance.
