Smith, Inc. <br />2) Change Order No. 1 - Miscellaneous <br />Intersections Phase 3-A Signalization <br />Signal Construction Company, Inc. <br />DATE: April 4, 1990 <br />DESCRIPTION AND CONDITIONS <br />On October 24, 1989, the Board awarded a construction <br />...contract in the amount of $213,683.51 to Dennis L. Smith, <br />:::Inc. for road widening improvements and a contract in the <br />amount of $86,340 to Signal Construction Co., Inc. for <br />,,:.;.signalization for the following intersection improvements: <br />8th Street at 43rd Avenue <br />8th Street at 20th Avenue <br />45th Street at 43rd Avenue <br />(signalization only) <br />The design was performed by Lloyd and Associates, Inc. The <br />construction began Nov. 13, 1989. <br />Almost immediately, it became apparent that there were <br />inconsistencies with the contract document bid schedules and <br />the amount of materials shown to be constructed on the <br />plans. Change Order No 1 to the Dennis L. Smith, Co.(DLS) <br />contract was recommended and approved on March 6, 1990 <br />increasing the DLS contract by $6,420 due to changes in <br />drainage design at 8th Street/43rd Avenue. <br />At this time, two additional change orders are being sub- <br />mitted as follows: <br />Change Order No. 2 to Dennis L. Smith Contract - This <br />Change Order increases the contract by $35,392.25 for <br />the following reasons: <br />1) The Engineer underestimated the quantity for <br />asphaltic concrete leveling course by 821 tons at <br />a cost of $31,132.25. This amount would have been <br />even greater if County staff had not changed the <br />pavement design during construction at 8th Street/ <br />20th Avenue. <br />2) The Engineer omitted adjustment of valve boxes on <br />8th Street/20th Avenue and new valve boxes were <br />recently installed at 43rd Avenue/8th Street. The <br />additional cost is $1,000. <br />3) The Public Works staff changed the slope -of the <br />road to -reduce asphalt quantities which resulted <br />in the need for a swale and grassing at a cost of <br />$2,500. <br />4) The Engineer omitted driveway replacement at 20th <br />Avenue/8th Street at a cost of $760. <br />65 <br />BOOK <br />
