Attachment A — SCOPE OF WORK <br />Project Name: <br />FIndian River County Artificial Reef <br />FWC Agreement No. <br />13127 <br />l <br />Construction 2013-2014 <br />reef material must be deployed in accordance with the specifications in this <br />Agreement. <br />ii. Documentation / Criteria Used as Evidence of Performance <br />1. A Materials Placement Report shall be submitted to the COMMISSION's <br />Project Manager within 30 days of field operations completion. The <br />Materials Placement Report form is available on the COMMISSION's <br />website at http://www.myfwc.comlconservation/saltwater/artificial-reefs/. <br />The Materials Placement Report must have a certification signature and <br />reflect an accurate material tonnage for the reef deployed as well as a <br />detailed description of the type, number, dimensions and individual weights <br />of the various sizes of reef materials deployed. This information may be <br />submitted on the materials placement report in lieu of taking loaded and <br />unloaded barge measurements. If accurate individual weights of concrete <br />units cannot be obtained or are not known, barge displacement measurements <br />are required. The GRANTEE's Project Manager or GRANTEE's designee <br />shall then record the waterline length, width and draft (to the nearest inch) of <br />the loaded barge at all four (4) corners to calculate the average displacement <br />of water due to the weight of the artificial reef materials. The same barge <br />measurements must be taken by either the GRANTEE's Project Manager or <br />GRANTEE's designee when the barge returns to shore after the deployment <br />has been completed. These measurements may not be taken while the barge <br />is offshore at the deployment site. The barge measurements are to be <br />included in the Materials Placement Report. <br />2. A final field report providing the designated observer's narrative, for each <br />day of the reef deployment operations, is required prior to reimbursement. <br />The field report shall include a written chronology and narrative describing <br />the deployment, and a performance evaluation of the marine subcontractors <br />who performed the work. The final field report should include photos of the <br />material and loaded barge prior to each deployment and, if available, <br />underwater photographs and video footage (surface and/or underwater). <br />iii. Timeline for Completion <br />All artificial reef construction must be completed by August 21, 2014. A one-time <br />modification for an extension of up to ten (10) days (through the Agreement end date <br />of August 31, 2014) may be authorized in writing by the COMMISSION's Contract <br />Manager under the following conditions: a) documented hazardous weather and sea <br />conditions do not allow for the safe deployment of reef materials offshore or, b) <br />unanticipated documented equipment malfunction on the transport vessel or <br />Version 1.0 — June 2013 Attachment A, Page 6 of 12 <br />130 <br />
