APPLICATION INSTRUCTIONS <br />The VOCA Grant Application packet consists of Parts 1-9 plus the required documentation. The <br />following information is provided to assist the applicant in completing the application packet. Late <br />applications will not be considered. Faxed or e-mailed applications will not be accepted. <br />For any questions concerning the VOCA Grant Application, contact the Bureau of Advocacy and Grants <br />Management, Office of the Attorney General, The Capitol, Room PL -01, Tallahassee, Florida 32399- <br />1050 at (850) 414-3380. <br />Part 1. - Applicant Information: Complete the information requested for the applicant agency. <br />Enter the agency name as it should appear on a contract in the event the program receives VOCA <br />funding. Note: An original signature is required on this form. <br />NEW MANDATORY REQUIREMENTS <br />The DUNS number is a unique nine -character number that identifies your organization. It is a tool of <br />the federal government to track how federal money is distributed. Most large organizations, libraries, <br />colleges and research universities already have DUNS numbers. Ask your grant administrator or <br />chief financial officer to provide your organization's DUNS number. If your organization does not <br />have a DUNS number, use the Dun & Bradstreet (D&B) online registration to receive one free of <br />charge. The website address for DUNS information/registration is: <br />http://fedgov.dnb.com/webform <br />Applicant agencies need to register your business with the federal government's SAM (formerly <br />Central Contract Registration), the primary database of vendors doing business with the federal <br />government. Register with SAM at: <br />http://www.sam.gov <br />Part 2. - Agency Eligibility: Complete all of the information requested. Use the listing of Florida's <br />judicial circuits to identify the circuit that the VOCA program will be serving (see page 9). For <br />example, Miami -Dade is in the 11th judicial circuit. Congressional District information can be found <br />at: <br />http://election.dos.state.fl.us/district-maps.shtmI <br />Part 3. -Certification Regarding Debarment, Suspension Ineligibility and Voluntary <br />Exclusion, Lower Tier Covered Transactions: This certification is required by federal regulations <br />implementing Executive Order 12549, Debarment and Suspension, 2B CFR Part 67, Section 67.510, <br />Participants= Responsibilities. The authorized representative at the subrecipient level must sign the <br />certification. The signed certification must be submitted with the grant application. <br />Part 4. - Funding Source Chart: Complete all of the information requested. <br />Part 5. - Statement of Need: Complete all of the information requested. <br />Part 6. - Project Proposal: Complete all of the information requested for each section. <br />Part 7. - VOCA Budget Request: The Budget section of the VOCA Application is an itemized <br />description by budget category of proposed costs for VOCA funding. The budget categories are <br />personnel, contractual services, equipment and operating expenses. Refer to the instructions on the <br />forms. <br />208 <br />Page 10 <br />
