Part 8. - Program Match Budget: The Final Program Guidelines require that all proposals provide a <br />20 percent match of the total VOCA project. Total VOCA Project is defined as the VOCA Budget <br />Request plus the Program Match. Match funds are subject to the same restrictions that govern <br />VOCA grant funds, i.e., the source of program match must be a VOCA-allowable expenditure. <br />Refer to the instructions on the forms. <br />Part 9. - Victims Served and Types of Services: Complete all of the information requested. <br />Required Documentation: <br />Related Parties Questionnaire: This form must be completed and signed by the Executive Director <br />or an agency official who is authorized to enter into contractual agreements. Additional narrative <br />pages may be attached if necessary. <br />Job descriptions: A job description is required for all proposed VOCA-funded personnel and match <br />personnel and must indicate the percentage of time allocated for each task totaling 100% of the job <br />duties. <br />Letters of Support: Attach three (3) current letters of support from local community or government <br />groups. A current letter is one that is dated during the current calendar year. It is the responsibility of <br />the applicant agency to ensure letters highlight the applicant agency's record of providing effective <br />services to victims of crime (if applicable), demonstrates the writer's support as well as the <br />community's support of the services that are requested as part of VOCA Grant project. Do not <br />provide more than three letters. The following will not be accepted: <br />* Letters from individuals that are not writing on behalf of a local community or government group <br />* Letters from individuals or units within the applicant agency will not be accepted <br />Letters acknowledging conference or meeting participation are not acceptable as letters of <br />* support <br />* Form letters <br />Documentation of the agency's 501(c) 3 ruling from the Internal Revenue Service: <br />Provide documentation to document the nonprofit status of the applicant agency. This may include: <br />* proof that the Internal Revenue Service recognizes the organization as being tax exempt under <br />501(c)(3) of the Internal Revenue Code; <br />* a statement from a state taxing body or state secretary of state certifying that the organization is <br />a nonprofit organization and that no part of the organization's net earnings may benefit any <br />private shareholder or individual; <br />* a certified copy of a certificate of incorporation or similar document establishing nonprofit status; <br />* any of the above, if it applies to a state or national parent organization, with a statement by the <br />state or national parent organization that the applicant is a local nonprofit affiliate <br />209 <br />Page 11 <br />
