R. Change Order #2 - IRC Health Clinic <br />The Board reviewed the following memo dated 9/12/90: <br />DATE: SEPTEMBER 12, 1990 <br />TO: HONORABLE BOARD OF COUNTY COMMISSIONERS <br />THRU: JAMES E. CHANDLER <br />COUNTY ADMINISTRATOR <br />FROM: H.T. "SONNY" DEAN, DIRECTOR <br />DEPARTMENT OF GENERAL SER <br />SUBJECT: INDIAN RIVER COUNTY HEALTH CLINIC <br />CHANGE ORDER NUMBER 2 <br />BACKGROUND: <br />The following Proposal Requests are hereby submitted on the subject <br />project which will necessitate a change in the contract with Miorelli <br />Engineering and Indian River County. <br />P.R. 4 - Change floor coverings in certain areas to accommodate <br />traffic. Delete a window. Upgrade door type to meet fire <br />code. Cost of $1,311.00 and no additional time. <br />P.R. 5 - Change Reinforced Concrete Pipe size from 12" x 18" to 14" <br />x 23" to accommodate stormwater. Cost of $276.00 and no <br />additional time. <br />P.R. 6 - Add asphalt overlay of 501 for ditch cut on 27th Street as <br />required by City of Vero Beach. Cost of $1,690.00 and no <br />additional time. <br />P.R. 7 - Reduce size of balconys to accommodate oak trees on site. <br />Credit of $2,067.00 and no additional time. <br />P.R. 9 - Upgrade of certain doors to meet code. Was not required by <br />code and request was rejected. No money or time. <br />P.R. 10 - Extend telephone conduit to south property line. Part of <br />original contract and was rejected. No money or time. <br />P.R. 12 -.Install water pipe for replenishment to HVAC system. Cost <br />of $373.00 and no additional -time. <br />ANALYSIS: <br />Net effect on the original contract would be an increase of $1,583.00 <br />or a total contract price of $2.432.333.00 and no additional -days. <br />Completion date remains at May 10, 1991.' <br />RECOMMENDATIONS: <br />Staff recommends Board approval of the Proposal Requests as <br />submitted, and requests authorization for the Chairman to execute the <br />formal Change Order. <br />SE21 <br />P 1 0 <br />
