CHANDLER <br />COUNTY ADMINISTRATOR <br />FROM: TERRANCE G. PINTO <br />DIRECTOR OF UTILIT ERVICES <br />PREPARED WILLIAM F. MCC <br />AND STAFFED CAPITAL PROJEC EER <br />BY: DEPARTMENT OF TY SERVICES <br />SUBJECT:FINAL PAY REQUEST AND CHANGE ORDER NO. 2 FOR 619, 8", <br />AND 12" WATER DISTRIBUTION MAINS ON 43RD AVENUE, <br />44TH AVENUE, AND 19TH STREET <br />INDIAN RIVER COUNTY PROJECT NO. UW -89 -16 -DS <br />BACKGROUND <br />....,On October 23, 1990, the Indian River County Board. of County <br />Commissioners approved a contract with Belvedere Construction for <br />the above -listed job. The project has been finalized out, and. we <br />are now prepared to make final payment. <br />ANALYSIS <br />The original contract -amount was $150,552.50. This was amended by <br />Change Order No. 1 for the construction of a water main ori 48th <br />Court (cost $14,998.00). The revised total was $165,550.50. <br />Attached is Change Order No. 2 (deduct of $872.25). This revises <br />the contract amount to $164,678.25. A detailed breakdown of Change <br />Order No. 2 is attached. The final pay request is in the amount of <br />$40,822.73, and it is also attached. <br />RECOMMENDATION <br />The staff of the Department of Utility Services recommends that the <br />Board of County Commissioners approve Change Order No. 2 and the <br />final payment of $40,822.73. <br />ON MOTION by Commissioner Scurlock, SECONDED by Com- <br />missioner Wheeler, the Board unanimously approved <br />Change Order No. 2 w/Belvedere Construction and final <br />payment of $$40,822.73 as recommended by staff. <br />MAY 14 1991 <br />57 <br />r'.UC <br />L <br />
