BRIDGE REPLACEMENTS AND WIDENING OF 66'"H AVENUE <br /> (W FROM SOUTH OF SR 60 TO NORTH OF 57TH STREET <br /> IRC PROJECT NUMBER 0545 <br /> PROFESSIONAL CIVIL ENGINEERING, GEOTECHNICAL ENGINEERING <br /> AND LAND SURVEYING SERVICES <br /> AMENDMENT NO. 13 TO THE PROFESSIONAL ENGINEERING/LAND SURVEYING <br /> SERVICES AGREEMENT BETWEEN ARCADIS US, Inc. AND INDIAN RIVER <br /> COUNTY,FLORIDA. <br /> This is an amendment to the existing Engineering/Land Surveying Services Agreement <br /> (AGREEMENT) dated September 6, 2005 between ARCADIS US, Inc. (ENGINEER) and <br /> Indian River County (COUNTY). This amendment addresses changes in "Section I — Project <br /> Limits and Description"and "Section V- Compensation". <br /> Amendment Description <br /> This Amendment includes the following: <br /> ".SECTION I-PROJECT LIMITS AND DESCRIPTION"is being modified to incorporate <br /> the following: <br /> Plans have beenP �r ared for the construction of 66'x'Avenue from south of SR 60 to north of <br /> 57a' Street. Included in the project is the construction of a 12 inch diameter water main on 33d <br /> Street.Also included is the construction of water main and reuse water main crossings at 33rd <br /> Street, 37' Street,45'x'Street, and 49`x' Street, and the relocation of an existing force main at 33rd <br /> Street. The COUNTY has requested that the ENGINEER revise the plans to extend the water <br /> main construction on 33rd Street an additional 500 feet to the west, and provide a final <br /> certification for these facilities to FDEP upon completion of the construction. <br /> In order to provide the construction completion certification, the ENGINEER will perform <br /> periodic inspections of the progress of the work. It is our understanding that the COUNTY will <br /> perform daily inspections of the contractor's operations to confirm that the utility lines are <br /> constructed in accordance with COUNTY standards. The ENGINEER will perform site <br /> inspections on a weekly basis during the installation of the utility lines to observe the contractors <br /> operations and confirm that the utility improvements are constructed in accordance with the <br /> project plans. In addition, the ENGINEER will attend construction progress meetings as <br /> requested by the COUNTY, and will provide engineering services on an as requested basis <br /> during construction to address questions and requests for additional information and to address <br /> unforeseen construction issues that may arise. The estimated hourly not to exceed fee included in <br /> this supplement is based on providing professional engineering services for six hours per week <br /> (two site visits per week) during a 16 week construction time period (16 hours Project <br /> (w Engineering Manager & 96 hours Sr. Field Coordinator).
