<br />depend, in part, on the number of valid claim forms thus far and subsequently received.

<br />A Claim IForm is attached. To be considered for payment, your Claim must be submitted online (at wwv
<br />MunicipalDerivativesSetdement.com), OR mail it by July 28, 2016. If you previously submitted a Claim Form, and you do no
<br />have any changes to make to it, you do not need to do anything—your previous Claim Form will be used for these settlement:

<br />from the Settlement Amount. At the Fairness Hearing, the Court will be asked to finally approve a plan, called a "Plan c
