■ Annual Statement of Compliance (within 60 days after the end of the calendar year), <br /> ■ Annual Operating Report(on or before April 1 of the following year), and <br /> ■ Results of an annual flare visible emissions test (within 4S days of testing). <br /> Effective December 31, 2013,the Major Air Pollution Source Annual Emissions Fee is calculated by <br /> FDEP's Electronic Annual Operating Report (EAOR) application that is used to produce the Annual <br /> Operating Report listed above.The emissions fee for 2016 is due by April 1, 2017. <br /> SCOPE OF SERVICES <br /> CONSULTANT will undertake the annual permit compliance monitoring and reporting,which will <br /> include the following tasks: <br /> Task 1: Project Quality Management <br /> Task 2: Semi-Annual Water Quality Sampling and Reporting <br /> Task 3: Title V Permit Compliance and Reporting; and <br /> Task 4: General Technical and Miscellaneous Permit Compliance Reporting <br /> The below Scope of Services is based on regulations and monitoring and reporting requirements as <br /> of the authorization date of this Work Order.An amendment to this Scope of Services may be <br /> needed if there are any regulatory changes that result in additional work. <br /> TASK 1.0—PROJECT QUALITY MANAGEMENT <br /> Activities performed under this task consist of those generally administrative functions required to <br /> assure that the project remains on schedule,within budget,and that the quality of the work <br /> products defined within this scope is consistent with CONSULTANT's standards and the COUNTY's <br /> expectations. <br /> CONSULTANT maintains a Quality Management System (QMS) on all projects.The CONSULTANT <br /> will comply with its QMS,which includes independent review of deliverables,monthly project <br /> status reviews,and project close-out activities.CONSULTANT will meet with COUNTY staff for <br /> project planning and coordination,as needed.CONSULTANT's project manager will attend status <br /> reporting meetings as needed throughout the life of the project. Preparation of invoices and project <br /> administration will also be performed under this task. <br /> TASK 2.0—SEMI-ANNUAL WATER QUALITY SAMPLING AND REPORTING <br /> CONSULTANT will assist COUNTY with all sampling preparation activities,including scheduling, <br /> staffing,subcontracting,and field equipment preparation.CONSULTANT will perform the field <br /> testing,sample collection,and water-level measurements. CONSULTANT will contract with Ideal <br /> Tech Services Inc. (ITS)to perform the groundwater and surface water sampling. Prior to collecting <br /> groundwater samples in January and July, 2017,CONSULTANT/ITS will collect water level data <br /> from 58 groundwater monitor wells and 1 staff gauge (SG-12).CONSULTANT/ITS will perform <br /> semi-annual sampling of the Class I and C&D debris landfill groundwater monitor wells in January <br /> (39 wells) and July (26 wells) of 2017. <br /> CONSULTANT/ITS will also perform semi-annual (January and July) sampling of the surface water <br /> site,if discharging. <br /> It is estimated that data collection and sampling will take four days to complete in January and <br /> three days to complete in July.
