INSTRUCTIONS TO PROPONENTS <br />B-1. PROPOSALS <br />B-1.1 Proposal Form <br />The Proposal Form for the operations contract and for the construction contract is bound in these <br />Contract Documents and shall not be removed therefrom. Proposal Forms must be completed in <br />ink. Contractor shall be required to state the cost of providing the operations services and of the <br />construction project portion of the work in this proposal form, and separately fill out related <br />construction documents in the "Class 1 Landfill Infill - Segments I and II" Contract Forms and <br />Specifications. <br />Proposals by corporations must be executed in the corporate name by the president or vice- <br />president (or other corporate officer accompanied by evidence of authority to sign) and the <br />corporate seal shall be affixed and attested by the secretary or an assistant secretary. The state of <br />incorporation shall be shown below the corporate name. Proposals by partnerships must be <br />executed in the partnership name and signed by a partner; title and the official address of the <br />partnership must be shown below the signature. <br />The names of all persons signing must also be legibly printed below the signature. A Proposal by <br />a person who affixes to his signature the word "president", "secretary", "agent", or other <br />designation without disclosing his principal relationship may be held to the Proposal of the <br />individual signing. When requested by County, evidence of authority of the person signing shall <br />be furnished. <br />All the blank spaces in the Proposal Form shall be filled. A proposal price shall be indicated for <br />each proposal item listed therein including Alternate Bid Item No. 1 or the words "No Charge", <br />"No Change", or other appropriate phrase shall be entered. Proposals received without all such <br />items completed will be considered non-responsive. Indian River County will select either the <br />Base Bid proposal for Class I Operations and Construction or the Alternate Bid Item No. 1 for <br />Class I Operations only or none of the above at their sole discretion. <br />Historical Leachate costs by Indian River County Landfill are found in Attachment C. <br />The Proposal shall contain an acknowledgment of receipt of all Addenda, the numbers and dates <br />which shall be filled in on the Proposal Form. <br />No alterations in Proposals, or in the printed forms by erasures, interpolations, or otherwise will <br />be acceptable unless such alteration be signed or initialed by the Proponent; if initialed, County <br />may require the Proponent to identify any alteration so initialed. <br />Terms used in these Instructions to Bidders which are defined in the Standard General <br />Conditions of this Contract have the meanings assigned to them in the General Conditions. <br />B-1 <br />bp0617/10-20.97 <br />•iiafit�`�J`';11�t 3�, ., <br />
