• Seven (7) children and three (3) adults have been terminated/aged out from the <br />program <br />• Five (5) children and two (2) adults met their goal <br />• Three (3) children were referred for other services <br />• Two (2) children and two (2) adult utilized other insurance <br />• All other outcomes are unknown at this time. <br />• Thirteen (13) referrals were not followed through on for treatment. <br />DASIE BRIDGEWATER HOPE CENTER, INC. <br />Dasie Hope Center Tutorial Program <br />(2015- 2016 — End of Year) <br />Grant: $30,000.00 <br />Funds Used as of: 10/15/2016: $30,000.00 <br />Total Children served as of: 10/15/2016: 127 (including summer camp) children <br />Cost per child: $236.22 <br />Focus Areas Addressed: Childcare/Child Health & Education <br />Taxonomy #: JR -820.650-45 Latchkey Safety Programs <br />Program Description: This program is designed to help educate and ensure the safety of <br />youth who normally would have to stay unsupervised in their homes after school or throughout <br />the day because their parent(s) are working. This program is designed to empower at -risk youth <br />with academics and social skills. Our priority is to "break the cycle of poverty" by sending kids <br />to college, improving their academics and behavioral thought patterns and surrounding them <br />with positive role models. <br />Collaboration: Indian River County School District, Disney Resort, Publix Supermarkets, <br />Indian River County Sheriffs Office Sea Oaks Community and Windsor community <br />CSAC Indian River County 2015-2016 Fourth Quarter Results <br />Goal #1: Increase in academic performance measured by grades on report cards and <br />assessments. 70% of students attending the tutorial program that receive services will perform <br />on grade level or above in math and English. Baseline: Students' academic record. <br />Outcome: Goal Met - 76% of youth performed at grade and above level. <br />Goal #2: Students will be mentored, provided supportive relationships and parents will be <br />involved in the academic process. 80% of students will show improvement in confidence, <br />learning ability, and decision-making measured by pre/post surveys. Baseline: Students' <br />2015-2016 Pre/Post surveys. <br />Outcome: Goal Met - 98% of youth have been pre and posted tested. <br />P175 <br />
