EXHIBIT A <br /> CCNA-2014 WO NO.10 <br /> FOR <br /> INDIAN RIVER COUNTY SOLID WASTE DISPOSAL DISTRICT <br /> C&D DEBRIS LANDFILL <br /> PERMIT RENEWAL AND COMPLIANCE WELL INSTALLATION <br /> This Authorization,when executed,shall be incorporated in and become part of the Continuing <br /> Contract Agreement for Professional Services between the Indian River County Solid Waste <br /> Disposal District(COUNTY),and CDM Smith Inc.(CONSULTANT),dated December 6,2011,and the <br /> Extension and Amendment of Continuing Contract Agreement for Professional Services,dated <br /> November 4,2014,hereafter referred to as the Contract. <br /> PROJECT BACKGROUND <br /> The current operations permit(Permit Number S031-0128769-021)for the construction and <br /> demolition(C&D)debris landfill was issued by the Florida Department of Environmental Protection <br /> (FDEP)on September 11,2012 (revised September 17,2012)and will expire on July 18,2017.This <br /> permit will need to be renewed at this time if the COUNTY intends to keep the C&D debris landfill <br /> operational.The permit renewal application is due on May 18,2017. <br /> The Scope of Work for preparing and submitting the renewal application are outlined below. <br /> SCOPE OF WORK <br /> TASK 1-C&D DEBRIS LANDFILL OPERATIONS PERMIT RENEWAL <br /> Subtask 1.1-Preparation of Permit Renewal Documents <br /> The"renewal'application consists of the four-page FDEP Form#62-701.900(6),Florida <br /> Administrative Code(F.A.C.)and other additional supporting information.In accordance with <br /> Chapter 62-701.730(2),F.A.C.,the renewal applications shall include the information in paragraphs <br /> (b)through(f).CONSULTANT will provide the following information as required in Part B of the <br /> application: <br /> Part 1- Not required for permit renewal. <br /> Part 2- A boundary survey and legal description and topographic survey of the property.A prior <br /> topographic survey(February 2012)will be used for this purpose.A new survey will not <br /> be performed as a part of this project. <br /> Part 3- An updated operations plan,which describes in detail the operations,safety,and training <br /> procedures,which are used on a daily basis to minimize the risk of environmental <br /> degradation.CONSULTANT will make modifications of the existing operating plan as <br /> required to incorporate the chromated copper arsenate(CCA)treated wood management <br /> plan and for mining of the C&D debris landfill to recover clean concrete and other <br /> recyclable materials. <br /> COM- <br /> SmithSmith A-1 )j1994.d= <br />
