■ This Work Order does not include any meetings with FDEP except the pre-application <br /> telephone conference. <br /> ■ Laboratory analysis for permit renewal will be performed by COUNTY's contract laboratory. <br /> DATA OR ASSISTANCE TO BE PROVIDED BY COUNTY <br /> ■ Existing data available on construction of the existing groundwater monitor wells. <br /> ■ A complete copy of the previous biennial report. <br /> ■ Relevant correspondence and documentation regarding operation of the C&D debris landfill <br /> during the last permit cycle. <br /> ■ Relevant correspondence and documentation regarding compliance with the applicable rules <br /> and regulations. <br /> ■ Available site surveys. <br /> ■ Available record information. <br /> ■ Laboratory analytical reports and direct access to laboratory personnel. <br /> ■ Application fee. <br /> PROJECT SCHEDULE <br /> The following project schedule has been developed based on receiving authorization on before <br /> March 21,2017. <br /> Task 1: CONSULTANT will participate in a pre-application telephone conference as soon as <br /> possible after receiving authorization.CONSULTANT will provide a draft copy of the <br /> permit renewal application for COUNTY to review by May 1,2017.Permit renewal <br /> sampling will be completed by May 1,2017.CONSULTANT will meet with COUNTY by <br /> May 10,2017 to review and discuss the comments on the draft permit renewal <br /> application.The final permit renewal application will be submitted to FDEP by May 17, <br /> 2017. <br /> PAYMENT AND COMPENSATION <br /> Compensation for this Work Order as described herein shall be made on the basis of a lump sum <br /> fee.The lump sum fee for Tasks 1 through 2,inclusive,is$30,420 as shown in Exhibit B. <br /> CONSULTANT will invoice COUNTY on a monthly basis based on percent complete.For invoice <br /> purposes only,the value of each task is as shown in the Table 1. <br /> Table 1 <br /> TASK VALUE FOR INVOICE PURPOSE <br /> TASK DESCRIPTION VALUE <br /> 1 C&D Debris Landfill Operations Permit Renewal $30,420 <br /> TOTAL LUMP SUM $30,420 <br /> CDM A-4 JJ1994.doca <br />
