Ennis P.E., County Engineer <br />SUBJECT: Work Order No. 02 — Andersen Andre Consulting Engineers, Inc. <br />CR512 Westbound Resurfacing (Roseland Road to US 1) and CR512 Eastbound <br />Resurfacing (Easy Street to US 1) — Testing Services <br />DATE: April 20, 2017 <br />DESCRIPTION AND CONDITIONS <br />On September 16, 2014, the Board of County Commissioners approved the annual agreement for <br />Professional Geotechnical Services, with Andersen Andre Consulting Engineers, Inc. for various services <br />including, but not limited to, field and laboratory construction materials testing in Indian River County. This <br />agreement was amended and renewed by the Board of County Commissioners on September 20, 2016. <br />The purpose of Work Order No. 2 is to provide laboratory testing and material engineering related services <br />for the resurfacing of CR512 which includes the dual westbound lanes from Roseland Road to US -1 of CR512 <br />and the eastbound lanes from Easy Street to US -1. Andersen Andre Consulting Engineer, Inc. submitted a <br />proposal outlined in the attached Work Order No. 2 and Scope of Services (Exhibit A) for a not -to -exceed <br />amount of $98,732.00. <br />FUNDING <br />Funding is budgeted in the Account No. 10921441-053360-15016 Secondary Roads/FDOT SCOP <br />Grant/CR512 Resurfacing (Easy Street/US-1/Roseland Road) in the amount of $98,732.00. <br />RECOMMENDATION <br />Staff recommends approval of Work Order No. 2 to Andersen Andre Consulting Engineers, Inc. authorizing <br />the above-mentioned professional services as outlined in the Scope of Services (Exhibit A) and requeststhe <br />Board to authorize the Chairman to execute Work Order No. 2 on their behalf for a not -to -exceed amount <br />of $98,732.00. <br />ATTACHMENTS <br />Work Order No. 2 <br />Scope of Services (Exhibit A) <br />DISTRIBUTION <br />Andersen Andre Consulting Engineers, Inc. <br />Deward Howard, Construction Coordination Manager <br />APPROVED AGENDA ITEM FOR MaV 2"d. 2017 <br />P146 <br />
