Baird, County Administrator <br />From: Vincent Burke, P.E., Director of Utility Services <br />Prepared By: Arjuna Weragoda, P.E., Capital Projects Manager j) <br />Subject: <br />Final Pay and Release of Retainage to R.J.Sullivan Corp on the Lime Slurry Inject <br />Project— UCP 2715 <br />Bid No: 2013028 <br />DESCRIPTIONS AND CONDITIONS: <br />On May 7, 2013 the Indian River County Board of County Commissioners (BCC) awarded the contract <br />for the Water Plants Lime Slurry Injection Project to R.J. Sullivan Corporation at a cost of <br />$2,549,000.00. Notice to proceed was issued on August 21, 2013 to R.J. Sullivan to start the work at <br />the North and South Water Treatment Plants. Change order No.1 to replace the acid tank at the South <br />RO plant was approved by the board on October 8th, 2013 for a lump sum amount of $189,527.00. <br />Change order No.2 to paint the west clearwell and the hardware modules for communication at the <br />North RO plant was approved by the board on March 18th, 2014 for a lump sum amount of <br />$50,155.96. Change order No.3 to construct a new 6 -inch concrete slab on top of the existing slab at <br />the South RO plant acid containment area was approved by the board on July 8th, 2014 for a lump <br />sum amount of $8,661.00. The addition of the three change orders resulted in a contract amount of <br />$2,797,343.96. <br />The project has been completed and Change Order No.4 is to make final adjustments to owner <br />direct purchase items and relocating two PH probes to decrease the total contract price by <br />$963,819.01 for a final contract price of $1,833,524.95. <br />R.J. Sullivan Corporation has been paid $1,739,831.69 to date, with $91,570.09 held in retainage <br />and $1,485.11 owed for relocating the PH probes. <br />ANALYSIS: <br />The improvements are now complete and R.J. Sullivan Corporation submitted Final Pay Request <br />No. 15 for $93,055.20, which includes retainage in the amount $91,570.09 plus the relocation of PH <br />probes in the amount $1,485.11 (See Attachment 1). The total cost of the project was $1,832,886.89. <br />The payment of $93,055.20 to R.J. Sullivan Corporation will release retainage and the County's <br />obligation to the Contractor. <br />58 <br />
