is <br />Page 2 of <br />PROJECT CONSULTANTS: <br />Carter Associates, Inc. — Patrick Walther Engineer of Record <br />McMahon Associates, Inc. — David Hill - Signal designer <br />UTILITIES <br />Florida Power and Light, Co. — FPL is going to raise existing distribution facilities on the <br />west side of 271h Avenue to accommodate new mast arms. FPL. wants two (2) west <br />mast arms to be grounded to their closest poles. Pay item notes have been added to the <br />plans to address this. <br />AT&T — Aerial Facilities can remain in place. <br />Indian River County Utilities <br />Comcast Cable Communications, Inc. — Comcast has not responded to repeated emails about <br />whether or not they need to relocate their facilities. Pat Walther to forward emails to <br />James Ennis regarding this. <br />Florida City Gas <br />Indian River County Telecommunications <br />Indian River County Traffic Operations <br />OTHER ISSUES <br />Y Maintenance of Traffic: <br />o Contractor shall submit a Maintenance of Traffic Plan to the Indian River County, Engineering <br />Division for approval. Must be signed and sealed by a Florida Professional Engineer. <br />o Traffic shall be maintained at all times. <br />o Access for school, businesses and residences shall be maintained at all times. <br />o School buses, sanitation & US Postal Service must be maintained. <br />o The Maintenance of Traffic Plan shall meet the FDOT Index 600, FY2017-2018 Edition and <br />shall be signed and sealed by a Florida PE.-- If work is to be performed at night there will be <br />fees associated with that for Engineering staff. Bidders should account forsuch fees in their <br />bid. <br />St Johns River Water Management District permit (ERP #93656-2) <br />Testing per- FDOT specifications. (Geotech firm TBD). Anderson Andre Consulting Engineers, Inc. <br />will be providing geotechnical and materials testing services. <br />No cross -sonic logging will be done unless drill shaft logs indicate possible integrity issue. <br />o Drill Shaft installation will require a pre -drill shaft installation meeting with the applicable <br />staff prior to these operations commencing. Contractor to account for this in theiractivities <br />schedule. <br />o A Drilled Shaft Installation Plan (DSIP) will be required to be submitted and approved by the <br />County prior to commencement of Drill Shaft Installation. <br />1. Equipment staging locations within the ROW shall be addressed in this plan <br />Curb ramps and Detectable Warnings in accordance with FDOT Index#304. Embedded Mots with full <br />length fins are required per IRC Standards. <br />Add endums if required —Bidders to review plan docs and provide comments to Purchasing as soon as <br />possible and within 2 business days following receipt of Pre -Bid Meeting minutes.
