ON MOTION by Commissioner Eggert, SECONDED by <br />Commissioner Adams, the Board unanimously approved <br />and authorized the Chairman to execute the Amendment <br />to Agreement with the State of Florida Department of <br />Natural Resources for the Round Island Park <br />Improvements, as recommended by staff. <br />AMENDMENT TO AGREEMENT <br />IS ON FILE IN THE OFFICE OF CLERK TO THE BOARD <br />WATER MAIN REPLACEMENT - PROJECT NO. 2 <br />CHANGE ORDER 2 AND FINAL PAY REQUEST <br />IXORA PARR, GLEN ACRES AND COORS SUBDIVISIONS, AND 18TH AVENUE <br />DATE: FEBRUARY 21 1993 <br />TO: JAMES E. UW -92 -18 -DS <br />CHANGE ORDER 2 AND FINAL PAY REQUEST <br />BACKGROUND <br />Construction of the subject project has been completed, and the <br />contractor, Driveways, Inc. of Titusville, Florida, has made <br />application for final payment. The pay request includes an increase <br />of $6,546.53 as reflected on Change Order 2 (see attached). The <br />cost increase -is primarily due to 1) adding a 2 -inch water line <br />extension at the Ixora Park subdivision to complete a loop system, <br />which added one jack and bore and a wet tap connection; 2) <br />additional 6 -inch water line was required on 18th Avenue to avoid a <br />utility conflict; and 3) an increase in the quantity of sod and <br />number of fittings used as compared -to the original estimate. <br />ANALYSIS <br />All Indian River County requirements have been met, and all testing <br />has been satisfactorily completed. The original contract price was <br />$47,161.84. Change Order 1 (see attached) revised the original <br />contract price to $59,472.28. The approved Change Order 1 added <br />18th Avenue as an additional water line project. This water line <br />project along 18th Avenue was in close proximity to Cooks <br />Subdivision, which is part of the subject project (18th Avenue was <br />in an area that continued to have low pressure problems within the <br />system). The final contract price is $66,018.81, which includes <br />Change Order 2. The contractor has previously been paid $44,576.63; <br />this leaves a remaining balance of $21,442.18. <br />27 <br />FEB 161993 BOOK 88 PAGE�6� <br />
