BOOK 93 F-nE 23 <br />101H COURT, SW WATER ASSESSMENT PROTECT - CHANGE <br />ORDERS & FINAL PAYMENT - TREASURE COAST CONTRACTING, <br />The Board reviewed the following memo dated 8/23/94: <br />DATE: AUGUST 23, 1994 <br />TO: JAMES E. CHANDLER <br />COUNTY ADMINISTRATOR <br />FROM: TERRANCE G. PI P116- <br />DIRECTOR OF UTILIT�VICES <br />PREPARED ROBERT O. WISEMEN, P.E. <br />AND STAFFED ENVIRONMENTAL ENGINEER <br />BY: DEPARTMENT OF UTILITY S VICES <br />SUBJECT: 10TH COURT, SW WATER ASSESSMENT PROJECT <br />CHANGE ORDERS NO. 1, 2, AND 3 <br />AND FINAL PAYMENT REQUEST <br />INDIAN RIVER COUNTY PROJECT NO. UW -93 -29 -DS <br />on May 17, 1994, the Indian River County Board of County <br />Commissioners approved and awarded the above-mentioned project to <br />Treasure Coast Contracting, Inc., to construct a water main <br />extension with all appurtenances to -provide service for all <br />residences on the subject road. Construction has been completed, <br />and the contractor has made application for final payment (see <br />attached change orders and payment request). <br />ANALYSIS <br />All Indian River County requirements have been met; final clearance <br />has been obtained from the Florida Department of Environmental <br />Protection. <br />The original contract price for this project was $22,811.20. <br />Change order numbers 1 and 2 were necessary to meet the requirements <br />of Public Works. In order to avoid cutting the pavement and because <br />of limited right-of-ways, the location of the water main was changed <br />to the opposite side of the road. This change necessitated more <br />sodding restoration. This additional sodding contributed to the <br />largest change in the dollar amount. Change Order No. 3 was <br />required to make the line more accessible for connection to Phase IV <br />Water Expansion assessment project. Change Order No. 3 will be paid <br />from Account No. 473-000-169-215.00 since it will benefit properties <br />to be assessed in the Phase IV water expansion program. <br />Change Order No. 1 $ 734.00 <br />Change Order No. 2 3,442.50 <br />Change Order No. 3 1,135.00 <br />Total $5,311.50 <br />These change orders adjust the contract price to $28,122.70. This <br />adjustment does not exceed the estimated preliminary project cost. <br />The previous payment made was $20,530.08 leaving a balance of <br />$7,592.62 including retainage. <br />RECOMMENDATION <br />The Department of Utility Services recommends approval of Change <br />Order Nos. 1, 2, and 3 and of the final payment request of <br />$7,592.62 as payment in full for services rendered to Treasure Coast <br />Contracting, Inc. <br />72 <br />September 6, 1994 <br />
