Parents as Teachers Program <br />Program Year 10/1/2016-9/30/2017 <br />4th Quarter Report <br />Grant: $30,000 <br />Funds used as of 9/30/17: $30,000 <br />Total Families served as of 9/30/17: 88 <br />Cost per child: Per 10/1/2016-9/30/2017, CSAC funds used per family was $340.91 <br />Taxonomy Number: PH -6100.3300 — Home Based Parenting Program <br />Program Description: Parents as Teachers is home based parenting education. The goal of the <br />evidenced —based Parents as Teachers (PAT) Program is to provide low-income parents with <br />skills to maximize their child's cognitive, social, and emotional development during the most <br />critical period of brain growth birth to three years. Staff will screen children regularly for <br />developmental delays and health issues. The program's long-term goal is to increase children's <br />readiness for kindergarten_ <br />Collaboration: Indian River Medical Center, Partners in Women's Health, Kindergarten Readiness <br />Collaborative, Moonshot Moment <br />Goals/ Outcomes Report (July 1, 2017- September 30, 2017) <br />Outcome # l: Increase parent's knowledge of their child's emerging development and age <br />appropriate child development by 95% Baseline: Program began in July 2015. The recorded <br />results from July 2015 to December 2015 was 85%. <br />Goal 4th quarter results: Overall 95.25% <br />96% of families completed a family centered assessment and goal plan within 90 days of <br />enrollment and updated every 6 months <br />93% of children will received a complete developmental screening within 90 days of enrollment <br />(or by 7 months of age for infants enrolled prior to age 4 months) and annually thereafter <br />96% of children will received a complete social -emotional screening within 90 days of <br />enrollment (or by 7 months of age for infants enrolled prior to age 4 months) and annually <br />thereafter <br />96% of children receive a hearing and vision test with a general health status update <br />Outcome # 2: Improve parents parenting capacity, and parenting practices and parent-child <br />relationships by 85% Baseline: Program began in July 2015. The recorded results from July 2015 <br />to December 2015 was 80%. <br />Goal 4th quarter results: Overall 94% <br />89% of parents demonstrate understanding the child's communication and cues and is responsive <br />in a positive manner <br />93% of parents develop a reading time with child <br />100% of parents demonstrate increased frequency, duration, and quality of parent-child <br />interactions <br />Outcome #3: 95% of target parents develop self -advocacy skills and obtain support that will lead their <br />family's success and self-sufficiency Baseline: Program began in July 2015.
