Goal 4th quarter results: Overall 58% <br />58% of families are connected to at least 1 community resource each year <br />58% of parents successfully connected to resources <br />TLC Newborn Program <br />Program Year 10/1/2016-9/30/2017 <br />4th Quarter Report <br />Grant: $12,000 <br />Funds used as of 9/30/17: $12,000 <br />Total Infants served as of 9/30/17: 943 <br />Cost per child: Per 10/1/2016-9/30/2017, CSAC funds used per infant $12.73 <br />Taxonomy Number: LJ -5000.1000 Breastfeeding Support Program <br />Program Description: TLC Newborn program promotes and encourages bonding activities of parents <br />with their newborn babies including successful principles of breast feeding, reading early to infant and <br />monthly newsletters of development milestones. TLC Newborn provides, parents with infant safe sleeping <br />practices to avoid accidental infant deaths and injuries. <br />Collaboration: Indian River Medical Center, Partners in Women's Health, Belly Beautiful, Kindergarten <br />Readiness Collaborative <br />Goals/ Outcomes Report (July 1, 2017 — September 30, 2017) <br />Outcome # 1: At least 94% of mothers who give birth at Indian River Medical Center will <br />receive education about quality preventive health care for infants. Baseline 93% reported <br />in 2015-2016. <br />Goal 3rd quarter results: Overall 100% <br />100% of clients received education and support around infant and baby health. <br />100% of clients received "this side up" onesie to promote safe sleep, prevent SIDS and <br />asphyxiation. <br />Outcome # 2: At least 86% of infants\children seen by TLC Newborn consultant will <br />develop early and appropriate healthy eating habits. Baseline: 82% reported in 2014-2015. <br />Goal 3rd quarter results: 88.00% <br />87.00% of parents chose to breastfeed their babies. <br />Outcome #3: 94 % of parents will articulate use of appropriate emotional/social responses <br />with infant to build a foundation of trust and security in child. Baseline: 90% reported in <br />2014\2015 <br />Goal 3rd quarter results: 100% <br />100% of parents articulate being able to understand specific baby communication with <br />appropriate positive responses. <br />-35- <br />48 <br />
