3. Proof of Ownership, e.g. recorded copy of Property Deed. <br />4. Current Property Tax Receipt. <br />5. Most current year's Tax Returns or current year's Tax Transcripts from IRS. <br />6. Paycheck Stubs (last 3 pay stubs for each working member, with additional <br />and updated stubs required at the County's discretion). <br />7. Most current Social Security Statement of Benefits for each household <br />member. <br />8. Most current documentation for other assets - 401(k), retirement/pension, <br />IRA, CDs, annuities, etc. <br />9. Most current self-employment income statement with schedule C, E, or F. <br />10. Documentation on dependents claimed (including birth certificate, school <br />records, court-ordered letter of guardianship, divorce decree and/or letter of <br />adoption). <br />11. Three most current bank statements for all open checking, savings or other <br />interest bearing accounts at the time of application and at contract signing. <br />12. Property Appraiser documentation showing what year the unit was built. <br />13. Third party contact information and release to obtain third party verification of <br />employment signed by employer and notarized. This documentation must be <br />received for eligibility determination. <br />14. DRI only: Documentation showing the Applicant was displaced or received <br />damage from Federally Declared Disaster. <br />The PA/HRS should review the application carefully to ensure that the Applicant <br />has provided all sources of income, including total household assets and asset <br />income. If an Applicant has trouble completing the program application and <br />providing the required back up documentation necessary needed for final <br />program eligibility. It may be preferable to schedule a meeting between the HRS <br />and Applicant to complete the application for assistance in a scheduled, face-to- <br />face interview where the Applicant can be prompted to provide accurate and <br />comprehensive income and household information. it is common for Applicants <br />to misunderstand what items to count toward annual income. <br />1. Applicant Responsibility: <br />Applicant Homeowners and all household members are responsible for <br />working in good faith and in a responsible, reasonable, and timely manner <br />with the HRS. Reasonable deadlines shall be established at each step of the <br />process of submittal of information by the homeowner. Generally after two <br />written notifications, any refusal or failure to provide information, or any <br />established pattern of untimely submittal of information by the homeowner, at <br />the HRS's discretion and approval by the PA the Applicant's case file may be <br />closed. <br />J.
