FWC Agreement No. 18101 <br />STATE OF FLORIDA <br />FLORIDA FISH AND WILDLIFE CONSERVATION COMMISSION <br />AGREEMENT NO. 18101 <br />CFDA Title(s): Federal Aid in Sportfish Restoration <br />CFDA No(s).: 15.605 <br />Name of Federal Agency(s): U.S. Fish and Wildlife Service <br />Federal Award No(s): FL -F -F 18AF01210 <br />Federal Award Year(s): 2018-2019 <br />Federal Award Name(s): Marine Artificial Reef Planning, Development, Administration, and <br />Assessment in Florida <br />CSFA Title(s).: Florida Artificial Reef Program <br />CSFA No(s).: n/a <br />State Award No(s).: FWC-18101 <br />State Award Year(s): n/a <br />State Award Name(s): Indian River County Artificial Reef Construction 2018-19 <br />This Agreement is entered into by and between the Florida Fish and Wildlife Conservation Commission, <br />whose address is 620 South Meridian Street, Tallahassee, Florida 32399-1600, hereafter "Commission," and Indian <br />River County Board of County Commissioners, FEID # 59-6000674, whose address is 1801 27th Street, Vero <br />Beach, FL 32960, hereinafter "Grantee." <br />WHEREAS, the Commission and Grantee have partnered together to construct two artificial reef patch reefs <br />by deploying at least 500 tons of secondary use concrete and 10 reef modules; and, <br />WHEREAS, Grantee has been awarded Indian River County Artificial Reef Construction Grant Agreement <br />number FWC-18101; and, <br />WHEREAS, such benefits are for the ultimate good of the State of Florida, its resources, wildlife, and public <br />welfare. <br />Now THEREFORE, the Commission and the Grantee, for the considerations hereafter set forth, agree as <br />follows: <br />1. PROJECT DESCRIPTION. <br />The Grantee shall provide the services and perform the specific responsibilities and obligations, as set forth <br />in the Scope of Work, attached hereto as Attachment A and made a part hereof (hereafter, Scope of Work). <br />The Scope of Work specifically identifies project tasks and accompanying deliverables. These deliverables <br />must be submitted and approved by the Commission prior to any payment. The Commission will not accept <br />any deliverable that does not comply with the specified required minimum level of service to be performed <br />and the criteria for evaluating the successful completion of each deliverable. If this agreement is the result <br />of Grantee responses to the Commission's request for competitive or other grant proposals, the Grantee's <br />response is hereby incorporated by reference. <br />GRANT - GOVERNMENTAL ENTITY Ver.
