Attachment A — SCOPE OF WORK <br />Project Name: <br />Indian River County <br />FWC Agreement No. <br />18101 <br />Artificial Reef Construction 2018-19 <br />i. Minimum Level of Performance <br />The artificial reef must be deployed within the boundaries of the permitted area and in <br />compliance with all the applicable permits and authorizations associated with the <br />permitted area. At a minimum, a total of at least 500 tons of clean concrete artificial <br />reef material and at least 10 reef modules must be deployed in accordance with the <br />specifications in this Agreement. <br />ii. Documentation / Criteria Used as Evidence of Performance <br />1. A Materials Placement Report shall be submitted to the COMMISSION's <br />Project Manager within 30 days of field operations completion. The Materials <br />Placement Report form is available on the COMMISSION's website at <br />http://www.myfwc.com/artificialreefs. The Materials Placement Report must <br />have a certification signature and reflect an accurate material tonnage for the <br />reef deployed as well as a detailed description of the type, number, dimensions <br />and individual weights of the various sizes of reef materials deployed. This <br />information may be submitted on the materials placement report in lieu of <br />taking loaded and unloaded barge measurements. If accurate individual <br />weights of concrete units cannot be obtained ,or are not known, barge <br />displacement measurements are required. The GRANTEE's Project Manager <br />or GRANTEE's designee shall then record the waterline length, width and <br />draft (to the nearest inch) of the loaded barge at all four (4) corners to calculate <br />the average displacement of water due to the weight of the artificial reef <br />materials. The same barge measurements must be taken by either the <br />GRANTEE's Project Manager or GRANTEE's designee when the barge <br />returns to shore after the deployment has been completed. These <br />measurements may not be taken while the barge is offshore at the deployment <br />site. The barge measurements are to be included in the Materials Placement <br />Report. <br />2. A table providing coordinates for each deployed module is required and is to <br />be attached to the Material Placement Report. Coordinates can be recorded by <br />the contracted vendor during deployment or by the GRANTEE during post - <br />deployment monitoring. <br />3. A final written field assessment providing the designated observer's narrative, <br />for each day of the reef deployment operations, is required prior to <br />reimbursement. The field report shall include a written chronology and <br />narrative describing the deployment, and a performance evaluation of the <br />marine subcontractors who performed the work. The final field assessment <br />should include photos of the material and loaded barge prior to each <br />deployment and, if available, underwater photographs and video footage <br />(surface and/or underwater). <br />iii.
