Attachment A — SCOPE OF WORK <br />Project Name: <br />Indian River County <br />FWC Agreement No. <br />18101 <br />Artificial Reef Construction 2018-19 <br />associated with Native American, early European, or American settlement are encountered at <br />any time within the project site area, the permitted project shall cease all activities involving <br />subsurface disturbance in the immediate vicinity of the discovery. The applicant shall contact <br />the Florida Department of State, Division of Historical Resources, Compliance Review Section <br />at (850)245-6333. Project activities shall not resume without verbal and/or written <br />authorization. In the event that unmarked human remains are encountered during the permitted <br />activities, all work shall stop immediately and the proper authorities notified in accordance <br />with Section 872.05, F.S. <br />F. Funds from this Agreement may not be expended on salaries, training, or parts replacement or <br />repairs to rented or contractor owned equipment. Documentation of expenses and survey <br />reports must be submitted with the closeout package in order for reimbursement to be made. <br />G. The GRANTEE agrees to acknowledge the role of the Federal Aid in Sport Fish Restoration <br />Program funding in any publicity related to this Agreement. <br />5. COMPENSATION AND PAYMENT <br />A. COST REUABURSEMENT <br />The GRANTEE shall be compensated for a maximum of $55,250.00 on a cost reimbursement <br />basis in accordance with the Cost Reimbursement Contract Payment Requirements as shown <br />in the Department of Financial Services, "Reference Guide for State Expenditures" publication. <br />The cost reimbursement requirements section of the Reference Guide is attached hereto and <br />made a part hereof as Attachment D. The GRANTEE shall provide a minimum matching <br />contribution as specified in Attachment B, Exhibit 1 and item 14(c) Federal Funds. <br />B. INVOICE SCHEDULE <br />A single final invoice may be submitted after completion of the deliverable and must be <br />received by the COMMISSION no later than October 15, 2019 to assure availability of funds <br />for payment. A timely reimbursement request following completion of actual field operations <br />is strongly encouraged. <br />C. TRAVEL EXPENSES <br />No travel expenses are authorized under the terms of this Agreement. <br />D. FORMS AND DOCUMENTATION <br />1. The GRANTEE shall submit a completed Materials Placement Report form along with <br />the invoice and request for payment. The Materials Placement Report form is available <br />on the COMMISSION's website at:-http://www.myfwc.com/artificialreefs. <br />6. MONITORING SCHEDULE <br />A. The GRANTEE agrees to allow the COMMISSION to conduct on-site inspections of the artificial <br />reef materials, staging area and construction site before, during, and after the deployment. <br />Version 1.0 — June 2013 Attachment A, Page 9 of 12 <br />
