a <br />Re: Notice of Random Drug Screening <br />In compliance with the Johnson -Davis, Inc. Drug -Free Workplace Policy, each <br />employee, as a condition of continued employment; is subject to medical or physical <br />examination or tests, including urine drug screen. An employee, who refuses to <br />submit to drug testing, as provided for in this Policy, will be asked to leave the office <br />or project site immediately and the employee will be discharged. <br />Please complete and sign the attached acknowledgement and authorization form <br />and return to your foreman and/or superintendent. You must proceed to a LabCorp <br />testing center in your local area upon receipt of this notice. (See superintendent for <br />LabCorp locations). <br />A copy of the Johnson -Davis, Inc. Drug -Free Workplace Policy is also attached for <br />your information and reference. Please keep, a copy of this policy along with your <br />employee handbook. <br />Thank you for your cooperation <br />
