Ennis, P.E., Assistant Public Works Director <br />FROM: Kirstin Leiendecker, P.E., Project Engineer <br />SUBJECT: Final Payment, Release of Retainage and Change Order No. 1 for <br />Indian River Boulevard Sidewalk from 37th Street to 53rd Street, IRC -1415 <br />DATE: January 28, 2019 <br />DESCRIPTION AND CONDITIONS <br />On March 6, 2018, the Board of County Commissioners awarded Bid No. 2018026 to Timothy Rose <br />Contracting, Inc. in the amount of $540,135.57 for the construction of a 5 -foot wide concrete sidewalk <br />along the west side of Indian River Boulevard from 37th Street to 53rd Street. The project also includes <br />pedestrian signals at Indian River Boulevard/ 37th Street intersection, Indian River Boulevard/ 41st Street <br />intersection, and Indian River Boulevard/ 45th Street intersection; associated drainage improvements; <br />earthwork, maintenance of traffic, erosion control and miscellaneous pavement markings. This project <br />is funded by a Florida Department of Transportation (FDOT) Local Agency Program (LAP) grant and will <br />provide up to $478,836.00 reimbursement of the eligible construction costs. <br />Change Order No. 1 is to make final adjustments to contract bid items resulting in a decrease to the total <br />contract amount by $.03 for a final cost of $540,135.54 and make final contract time adjustments. <br />Timothy Rose Contracting, Inc. has successfully completed the project and has been paid $496,593.56 <br />with $26,136.50 held in retainage. Timothy Rose Contracting, Inc. has submitted Contractor's <br />Application for Payment No. 6 for final payment of $17,405.48 and release of retainage in the amount of <br />$26,136.50 for a total of $43,541.98. Once final release of liens from all Timothy Rose Contracting, Inc. <br />subcontractors have been received, reviewed and approved, payment will be processed. <br />FUNDING <br />Funding in the amount of $43,541.98 is budgeted and available from the following accounts: <br />Optional Sales Tax <br />IR Blvd Sidewalk (37th — 53rd) <br />Account No. 31521441-066510-16025 <br />$17,405.48 <br />Optional Sales Tax — Retainage <br />Account No. 315-206000-16025 <br />IR Blvd Sidewalk (37th — 53rd) <br />$26,136.50 <br />29 <br />C:\Users Uegistar\AppData\Local\Temp\BCL Technologies\easyPDF 8\\@BCL@200E5 C99\@BCL@200E5C99.doc <br />
