QUARTERLY DELIVERABLES: <br />Quarterly deliverables must be uploaded into the Division's SharePoint portal and/or WebEOC each <br />quarter as stated in the scope of work. The Recipient must also provide a certification of completion on <br />the deliverable checklist. <br />NOTE: If there are no updates, duplicate information is not required throughout the period of <br />performance and is not required to be uploaded into the Division's SharePoint portal. Please just <br />notate such in the notes section on the Deliverables Checklist in SharePoint. <br />QUARTER 1: JULY 1, 2019 — SEPTEMBER 30, 2019 <br />In order to demonstrate successful completion of all required tasks for Quarter 1, the Recipient must <br />submit the following items in the Division's SharePoint portal and/or WebEOC: <br />Task 1 <br />• The FDEM Exhibit 2, certification letter for the full-time Emergency Management Director or part- <br />time Coordinator, in accordance with Rule 27P-19.004, Florida Administrative Code; <br />• The FDEM Form 4 — Staffing Detail and position descriptions for funded emergency management <br />staff, including an identification for which employees are responsible for Preparedness, <br />Response, Recovery, Mitigation, and Finance; <br />• A quarterly report as outlined in Quarterly Reports (Attachment D); <br />• The full-time Emergency Management Director's or part-time Coordinator's certified timesheets or <br />paystubs; <br />• Training certificates for all employees of the County Emergency Management Agency for any <br />G/IS, FEMA or Division courses that pertain to Preparedness, Response, Recovery, Mitigation <br />and Finance (if uploaded into SERT TRAC, the Division's Grant Manager will request a copy of <br />the transcript); <br />Task 2 <br />• The FDEM Form 3 - Local Budget Match Requirement; <br />• A copy of the current and accurate County Emergency Management Local Budget (General <br />Revenue) including the budget approval date. All requests for a budget match reduction shall be <br />requested no later than 45 days after the county budget has been approved; <br />Task 3 <br />• Support of Response capabilities (Attachment E), including contact information for emergency <br />management agency and other contacts, County Director and alternate contacts, State Mutual <br />Aid Agreement contacts and others; <br />• Site Data including location and attribute information for all fire rescue, law enforcement, public <br />safety, and emergency service stations; <br />• Site Data including location and attribute information for all pre -identified sites for the County <br />Staging Areas (CSAs) and Points of Distribution (PODs); <br />• Updates applicable to Hurricane Shelter Retrofit (Attachment F) in accordance with Florida's <br />statewide hurricane shelter space deficit elimination program; <br />Task 4 <br />• Support of Recovery capabilities (Attachment G), identify potential Disaster Recovery Center <br />(DRC) locations and provide basic information in WebEOC. <br />25 <br />
