QUARTER 2: OCTOBER 1, 2019 — DECEMBER 31, 2019 <br />In order to demonstrate successful completion of the required tasks for Quarter 2, the Recipient must <br />submit the following items in the Division's SharePoint portal and/or WebEOC: <br />Task 1 <br />• Any updates to the FDEM Form 4 — Staffing Detail and position descriptions for funded <br />emergency management staff, including an identification for which employees are responsible for <br />Preparedness, Response, Recovery, Mitigation, and Finance; <br />• A Quarterly Report as outlined in Quarterly Reports (Attachment D); <br />• The full-time Emergency Management Director's or part-time Coordinator's certified timesheets or <br />paystubs; <br />• Any new certificates obtained by employees of the County Emergency Management Agency for <br />any G/IS, FEMA or Division courses that pertain to Preparedness, Response, Recovery, <br />Mitigation and Finance (if uploaded into SERT TRAC, the Division's Grant Manager will request a <br />copy of the transcript); <br />Task 2 <br />• Any updates to the FDEM Form 3 - Local Budget Match Requirement; and <br />• Any updates to the current and accurate County Emergency Management Local Budget (General <br />Revenue) including budget approval date; <br />Task 3 <br />• Updates to support of Response capabilities (Attachment E), including contact information for <br />emergency management agency and other contacts, County Director and alternate contacts, <br />State Mutual Aid Agreement contacts and others; <br />• Updates for Site Data including location and attribute information for all fire rescue, law <br />enforcement, public safety, and emergency service stations; <br />• Updates for Site Data including location and attribute information for all pre -identified sites for the <br />County Staging Areas (CSAs) and Points of Distribution (PODs); <br />• Updates applicable to the Hurricane Shelter Retrofit (Attachment F) in accordance with Florida's <br />statewide hurricane shelter space deficit elimination program; <br />Task 4 <br />• Updates to support Recovery capabilities (Attachment G), identify potential Disaster Recovery <br />Center (DRC) locations and provide basic information in WebEOC. <br />QUARTER 3: JANUARY 1, 2020 — MARCH 31, 2020 <br />In order to demonstrate successful completion of the required tasks for Quarter 3, the Recipient must <br />submit the following items in the Division's SharePoint portal and/or WebEOC: <br />Task 1 <br />• Updates to the FDEM Form 4 — Staffing Detail and position descriptions for funded emergency <br />management staff, including an identification for which employees are responsible for <br />Preparedness, Response, Recovery, Mitigation, and Finance; <br />• A quarterly report as outlined in Quarterly Reports (Attachment D); <br />• The full-time Emergency Management Director's or part-time Coordinator's certified timesheets or <br />paystubs; <br />• Any new certificates obtained by employees of the County Emergency Management Agency for <br />any G/IS, FEMA or Division courses that pertain to Preparedness, Response, Recovery, <br />Mitigation and Finance (if uploaded into SERT TRAC, the Division's Grant Manager will request a <br />copy of the transcript); <br />26 <br />
