MIK SERVICES, INC. <br />EXCLUSIONS <br />Storage <br />Foundations <br />Utility supply <br />PAYMENT TERMS: 90% NET 30 Days, 10% Retainage up to a maximum of 120 days. <br />SHIPMENT: Shipment can be made 11-13 weeks after receipt of final drawing <br />approval and release for manufacture. All shipments are F.O.B. <br />factory with full freight allowed to the jobsite. All claims for damage or <br />loss during shipment shall be initiated by the purchaser. <br />SERVICE: Field service is included as specified and called out above. <br />TAXES: This proposal does not include any sales or use taxes of any kind. <br />VALIDITY: This proposal is firm for 60 days after which it is subject to withdrawal <br />or change without notice. <br />INSTALLATION: Installation will require four (4) weeks after all materials are delivered <br />to the jobsite.
