EXHIBIT "A" <br />(1) Copies of UTILITY's franchise issued b1 the COUNTY and <br />presently in effect relating to the operation of the <br />UTILITY system. <br />(2) Copy of the Tariff and Uniform Extension Policy of UTILITY. <br />(3) Schedule of rates, fees and charges in effect at the time <br />of this Agreement. <br />(A) Copies of UTILITY's "Annual Report" for the last two (2) <br />years as filed with the COUNTY. <br />(5) Map of the area authorized by the franchise to be served <br />by UTILITY. <br />(6) Plans and Specifications, including profiles, showing the <br />sewage treatment plants of the utility system as now <br />constructed (as -built), together with a map showing the <br />sewage collection lines and irrigation water distribution <br />lines and appurtenances as now constructed (as -built), and <br />all other facilities constituting the sewer system (within <br />thirty (30) days after contract execution). <br />(7) Plans, specifications and profiles of certain sewage <br />collection lines and irrigation water distribution under <br />construction as of the date of the Agreement, if any. <br />This Exhibit will be amended at closing to add such <br />facilities the construction of which has commenced after <br />the date of this Agreement. <br />(8) Copies of permits, applications or other documents, <br />together with effective dates and expiration dates (i£ <br />any), demonstrating approval of the facilities of the <br />utility system by all applicable governmental authorities, <br />including Florida Department of Environmental Regulation <br />or its regulatory predecessor, Indian River County Health <br />Department and COUNTY. <br />9) Inventory of all equipment, vehicles, tools, parts, <br />laboratory equipment, office equipment, unset or reserved <br />meters, and other personal property located on or in <br />connection with the property being acquired by COUNTY <br />pursuant to this Agreement and that UTILITY uses in normal <br />operation of the utility system, together with a schedule <br />showing the nature of the ownership or use rights thereof <br />of UTILI'T'Y. With respect to this personal property, <br />UTILITY represents that same will not be depleted prior to <br />the closing, except in the normal and ordinary course of <br />the operations of the systems by the UTILITY. <br />(10) A description of all private easements and right of way <br />owned or legally used by UTILITY for the construction, <br />operation and maintenance of the utility system, all of <br />which are transferrable to COUNTY. In this regard, maps <br />showing the location of such easements, with true copies <br />of the Easement Agreement containing the legal <br />descriptions of the lands covered by the easements, will <br />suffice. Easements located or shown in recorded plats and <br />rights to locate lines in dedicated public rights of way <br />need not be included in this Exhibit. <br />(11) A schedule of all customers' advances held by UTILITY for <br />construction, including the amounts thereof, the parties <br />who made the advances, the status of the construction <br />work, and the purposes for which the customer advances <br />[EXHIBI <br />1 "A" <br />
