6. Accident/ Incident/ Occurrence Investigation & Reporting <br /> This chapter defines what is considered an accident. It also outlines.the actions to be taken by <br /> employees in the event of an accident/incident related to Senior Resource Association vehicles, <br /> passengers, employees or property in the course of doing business. <br /> The definition of an accident is an event involving any of the following: (Accident/Incident report <br /> required) <br /> • Loss of life <br /> • Serious injury to a person <br /> • Collision involving a transit vehicle <br /> • An evacuation for life safety reasons <br /> The definition of an incident is an event involving any of the following: (Accident/Incident report <br /> required) <br /> • Personal injury that is not serious <br /> • One or more non-serious injuries requiringmedical transport <br /> • Damage to the facilities equipment or infrastructure that disrupts the operations of the agency <br /> The definition of an occurrence is an event involving any of the following: (Accident/Incident <br /> report required) <br /> • An event without any personal injury in which any damage to facilities, equipment,or <br /> infrastructure does not disrupt operations <br /> The definition of a serious injury is an event involving any of the following: (Accident/Incident <br /> report required) <br /> •Requires hospitalization for more than 24 hours <br /> •Results in a fracture of any bone <br /> •Causes severe hemorrhages,nerve,muscle, or tendon damage <br /> • Involves any internal organ; or Involves any second-or third-degree burns <br /> 103 <br />
