Preliminary Accident Reports: (Preliminary Accident reports will be sent as soon as possible <br /> after the accident, but not later than three hours of clearing the accident scene). All reports <br /> will be scanned& sent to the Safety& Training Department. Required documents include <br /> accident/incident reports, pictures, customer comment cards or witness statements,police report, <br /> refusal of medical treatment forms, Drug and alcohol tests (if required) and any other available <br /> information. <br /> NOTE: Accident/Incident reports (employee& investigator) are required to be completed <br /> and turned in prior to the end of the employee's shift(of the day of the event). <br /> Failure to comply with the Accident/Incident reporting procedure and submission <br /> deadlines,outlined above,WILL result in progressive disciplinary action. Furthermore,the <br /> operator williberemovedEfrom theiFiliiil schedule until such time as the required <br /> paperwork is completed and turned in to the Safety and Training Department. <br /> Accident/Incident Final Report: Should be completed and turned in to Department Heads and <br /> HR within 5 days of the accident/incident. The Safety department will be responsible for <br /> submitting the final report. <br /> Determinations and Point System <br /> Using the accident/incident final report,the accident/incident determination form will be filled <br /> out and scored. The Cumulative violations of four(4)points may result in possible termination. <br /> Each preventable accident/incident will not be held against the employee after 365 calendar days <br /> has passed (i.e. if an employee receives a preventable accident/incident on June 1, 2019, it will <br /> not be effective after June 1, 2020. <br /> 105 <br />
