5310 Vehicles <br /> SRA is required to notify their FDOT district office within 24 hours of any accident involving a <br /> 5310 funded vehicle in which FDOT is the lien holder. Following information is required: <br /> 1. Date of the accident. <br /> 2. Confirmation that SRA conducted an accident investigation in accordance with System <br /> Management System (SMS). <br /> 3. A copy of the accident report. <br /> Near-Miss Reporting <br /> The Senior Resource Association SMS requires proactive reporting of safety hazards or safety <br /> concerns on the part of all employees in order to maintain a proactive position on risk. <br /> Employees will serve as Safety Ambassadors by working safely, complying with requirements <br /> and serving as an example to others. <br /> Under no circumstance will employees be retaliated against for the act of reporting safety <br /> related information. <br /> The Senior Resource Association SMS contains both proactive and reactive means of reporting <br /> safety hazards and near-miss events. The information derived from proactive reports can provide <br /> significant insights into potential risk(s)and furthermore, allows for the continuous improvement <br /> of Senior Resource Association's SMS. While post-event accident/incident investigations reveal <br /> information about safety hazards,we cannot rely solely on reactive data to expose risk. <br /> Process and Procedure: <br /> When an employee becomes aware of a hazard or near-miss,they shall submit a report within <br /> eight(8) business hours or at end of shift. This can be done by submitting accident/incident <br /> reports with "near miss" indicated to supervision/management or place in the safety drop box. <br /> Supervision/management is then required to forward to Safety&Training supervisor. Reports <br /> may be entered anonymously if desired,where there is no record of who created and submitted <br /> the report <br /> When filing anonymous reports,there is limited ability to provide follow-up communication <br /> as the reporter is unknown.Additionally, it is difficult to apply necessary risk mitigation <br /> strategies if the reporter does provide enough information for follow-on activities. <br /> Therefore, it is recommended that reporters utilize the non-anonymous hazard reporting form. <br /> 111 <br />
