B. Organizational Capability <br />1. Provide the mission statement and vision of your organization: <br />Mission Statement: <br />New Horizons of the Treasure Coast and Okeechobee delivers accessible behavioral and primary health care services to children, <br />adults and families to achieve mental and physical wellness, thereby improving the quality of fife in our community. <br />Vicinn <br />New Horizons of the Treasure Coast and Okeechobee will become the preferred provider by having engaged employees delivering <br />evidence based and best practices, and by expanding services in response to the needs of the community. <br />2. Provide a brief summary of your organization, including areas of expertise, accomplishments and <br />New Horizons began as a Mental Health clinic more than 60 years ago, providing mental health and substance abuse services for <br />children, adults and their families, New Horizons provides a wide array of behavioral healthcare services for residents of Indian River <br />County ranging from community support services such as housing, case management and psychosocial rehabilitation; as well as <br />psychiatric services, therapy,. Inpatient crisis stabilization, and detoxification units. These services are made available to county <br />residents based on need regardless of their ability to pay. <br />New Horizons provides quality services, some of which are evidence based or best practice models. Services are measured for <br />effectiveness and meaningfulness to the client and their families and there is a continuous quality improvement model that Is followed <br />and reviewed. <br />Specifically in Indian River County, New Horizons opened the Indian River Learning Center in 2016 with the help of an Impact 100 <br />grant. The Learning Center is designed to provide psychosocial rehabilitation to those challenged with severe mental health Issues. <br />The Center offer recovery -oriented services to assist individuals on their road to recovery. They learn skills needed for independent <br />living, social skills and the use of community resources. In addition, New Horizons has responded to the Opioid Crisis by offering <br />quality medication assisted treatment. <br />New Horizons is licensed by the Agency for Health Care Administration (AHCA), Department of Children and Families (DCF) and is <br />accredited by the Commission on Accreditation of Rehabilitation Families (CARF). <br />3. Briefly list any certifications and/or accreditations obtained by your agency. <br />New Horizons currently manages 26 programs funded by the Florida Department of Children & Families (through Southeast Florida <br />Behavioral Health Network) and/or the Agency for Health Care Administration to provide mental health and substance abuse <br />services.
