❑ (d) Name, certificate of authorization number, and street and mailing <br />address of the business entity on the map and report. <br />❑ (e) Name and license number of the surveyor and mapper in <br />responsible charge. <br />❑ (f) Name, license number, and street and mailing address of a <br />surveyor and mapper practicing independent of any business entity <br />on the map and report. <br />❑ (g) Survey date (date of data acquisition.) <br />❑ (h) Revision date for any graphic revisions (when survey date does <br />not change.) <br />❑ (i) Map and report statement "Survey map and report or the copies <br />thereof are not valid without the signature and the original raised <br />seal of a Florida licensed surveyor and mapper." <br />❑ 0) Insurance statement in 1/4" high letters "The survey depicted here <br />is not covered by professional liability insurance" if there is no <br />professional liability insurance. <br />❑ (k) Additions or deletions to survey maps or reports by other than <br />the signing party or parties is prohibited without written consent of <br />the signing party or parties. <br />❑ (1) All computed data or plotted features shown on survey maps <br />supported by accurate survey measurements unless clearly stated <br />otherwise. <br />❑ (m) Bearings, distances, coordinates, and elevations shown on a <br />survey map shall be substantiated by survey measurements unless <br />clearly stated otherwise. <br />❑ (n) Bearing reference (well established and monumented line) <br />❑ (o) A designated "north arrow" <br />❑ (p) Stated scale or graphic scale <br />❑ (q) Abbreviations in legend or notes. <br />❑ (r) Special conditions and any necessary deviation from the <br />standards noted upon the map or report. <br />❑ (s) Responsibility for all mapped features stated on the map or report <br />❑ (t) Map or report clearly states the individual primarily responsible <br />for the map or report when mapped features have been integrated <br />with others. <br />(u) Map Accuracy. <br />(1) Vertical Feature Accuracy: <br />❑ (a) Vertical Control: Field -measured control for elevation information <br />shown upon survey maps or reports shall be based on a level loop or <br />closure to a second benchmark. <br />❑ (b) Closure in feet must be accurate to a standard of plus or minus <br />.05 ft. times the square root of the distance in miles. <br />Appendix A -As -Built Spec . <br />F:\Public Works\6NGINEERING DIVISION PROJECTS\1760 Victor Hart Sr. Complex Drainage Improvements\l-Admin\Bids\Bid <br />Documents\IRC-1760 APPEN A 20180810.doc <br />
