Mr. Scott Reynolds <br />March 17, 2021 <br />Page 2 <br />Tetra Tech recommended additional investigation for the Site, including investigation of <br />potential asbestos containing material (ACM), lead-based paint (LBP), and mold. <br />The Site has five artesian wells and one community water system well on Site. Consumptive <br />Use Permits have been received for the Site converting ownership to IRCDUS from the previous <br />owner. <br />SCOPE OF WORK <br />Florida Power and Light (FPL) intends to use the fenced in area of the Site as a construction <br />laydown area for work in the vicinity. Tetra Tech has been asked to abandon the public supply <br />well and artesian well inside the fenced area and complete the investigation of the arsenic in the <br />soils within the fenced area. <br />Task 1- Mobilization - $1,322.64 <br />Upon notice to proceed, Tetra Tech will conduct the following activities: <br />• Update the Health and Safety Plan (HASP); and <br />• Procure Subcontractors Necessary for Project Completion (Licensed Well Driller, <br />Florida, laboratory, and equipment). <br />Tetra Tech will update the Site -Specific Health and Safety Plan in accordance with OSHA <br />HAZWOPER requirements outlined in 29 CFP 1910 to include the newly added well <br />abandonment task. This plan update will be prepared by a health and safety professional under <br />the supervision of a Certified Industrial Hygienist. <br />Technical specifications and subcontract agreement for required subcontracted work will be <br />prepared under this task. These specifications will include, well abandonment, laboratory <br />analysis of soil samples, and equipment. <br />Tetra Tech will obtain all state and district permits the well abandonment under this task, as <br />needed. This Task will be, completed on a fixed fee basis in accordance with the provided <br />assumptions. <br />Task 2 — Well Abandonment - $18,078.42 <br />Tetra Tech and the subcontracted Licensed Well Driller will be on site to properly abandon two <br />wells located within the fenced area at the Premier Citrus Site. This includes: <br />• Abandonment of the 2 -inch diameter 75 -foot deep commercial supply well <br />• Abandonment of the 4 -inch diameter 735 -feet deep artesian agricultural supply well with a <br />casing set at 250 feet deep. <br />• The well shall be abandoned in accordance with State of Florida requirements. <br />• If access to the well requires removal of the roofed structure and or shed, means to remove <br />these items will not require a demolition permit, and removal of materials associated with the <br />structures from the site will not be required. <br />• Restoration of the well pad areas shall be compacted clean sandy fill. <br />TETRA 'TECH <br />
