Benefit Program Participation: <br /> Affirmations <br /> Please be aware that when an employee participates in SDIRC's benefit programs, <br /> the employee is automatically making the following affirmations: <br /> • Employee authorizes SDIRC to deduct payroll premiums for employee benefit <br /> elections and employee authorizes the deduction of any missed premiums not <br /> deducted from payroll for any reason. Employee acknowledges that employee will be <br /> responsible for any and all premiums, deductibles and copays that may apply. <br /> • Employee certifies that the information provided on the Explain My Benefits (EMB) <br /> enrollment portal is true and correct to the best of employee's knowledge. <br /> • Employee acknowledges that employee cannot stop or change benefits paid on a <br /> pre-tax basis during the plan year unless employee experiences a Qualifying Event or <br /> during the Open Enrollment period. <br /> • Employee agrees that SDIRC and its third party administrator are not responsible for <br /> employee's failure to read or understand all rules or regulations pertaining to <br /> benefits enrollment, nor employee's failure to enroll online accurately or to submit <br /> timely elections. <br /> • Employee agrees for employee and covered members of employee's family under <br /> District insurance plan(s) to be bound by the benefits, deductibles, copayments, <br /> exclusions, limitations, eligibility requirements and other terms of the plan contracts, <br /> agreements or plan documents for the plan(s) in which employee enrolls. <br /> • Employee agrees that they are exclusively responsible for and assume the risks <br /> associated with the choice of plan option(s) and covered dependents, selected by <br /> the employee, and agrees neither SDIRC or its representatives, employees, agents or <br /> insurers are responsible for choosing or providing advice on the plan options <br /> available for employee and employee's family. <br /> • Employee agrees that employee is responsible for reading, understanding, and <br /> asking questions regarding benefits, exclusions and limitations for each plan. Failure <br /> to adequately review employee plan options will not be a valid reason for a coverage <br /> change once Open Enrollment or New Hire Enrollment concludes.
