Important Legal Notices <br /> Family Medical Leave Act(FMLA) <br /> What does the Family and Medical leave act provide? <br /> The Family and Medical Leave Act (FMLA) provides eligible employees up to 12 work weeks of unpaid leave a year, and <br /> requires group health benefits to be maintained during the leave as if employees continued to work instead of taking leave. <br /> Employees are also entitled to return to their same or an equivalent job at the end of their FMLA leave. <br /> Who can take FMLA leave? <br /> To be eligible to take leave under FMLA an employee must: <br /> • Have worked 1,250 hours during the 12 months prior to the start of the leave (Note: Full-time teachers and other exempt <br /> employees are assumed to have worked 1,250 hours unless proven otherwise),and <br /> • - Have worked for the employer for 12 months(in total,not consecutive)within the last 7 years. <br /> When can an eligible employee use FMLA leave? <br /> A covered employer must grant an eligible employee up to a total of 12 workweeks of unpaid,job-protected leave(26 weeks in the <br /> case of military caregiver leave described below)in a 12 month period for one or more of the following reasons: <br /> • For the birth of a child: <br /> • For the placement with the employee of a child for adoption or foster care; <br /> • To take medical leave when the employee is unable to work due to a serious health condition; <br /> • To care for an immediate family member(spouse,child or parent-but not parent"in-law")with a serious health condition; <br /> • To care for a spouse,son,daughter,parent or next-of-kin on covered active duty service with a service-related serious health <br /> condition or injury; <br /> • To deal with a qualifying emergency arising from a son's, daughter's, spouse's or parent's (but not parent "in-laws") active <br /> duty service or call to active duty service for deployment to a foreign country. <br /> Responsibilities to the District Employees Requesting Leave. <br /> It is the responsibility of the employee to notify their supervisor and iContact: Amy Yeitter,Employee Benefits Specialist <br /> provide at least thirty(30) days notice before the date the FMLA leave is <br /> to begin if the need for the leave is foreseeable.If the need for the leave Address: 6500 57th Street,Vero Beach,FL 32967 <br /> is not foreseeable,you must give notice that you need to take a leave ofPhone:7 772-564-3175 <br /> absence as soon as practicable, but in no circumstances later than the <br /> next business day after you become aware of the need for the leave,.
