D. , <br />ENVIRONMENTAL ENGINEER <br />DEPARTMENT OF UTILITY SERVICES <br />WATER EXPANSION PLAN - PHASE V, CONTRACT 1 <br />CHANGE ORDER NO. 1 AND FINAL PAY REQUEST <br />INDIAN RIVER COUNTY PROJECT NO. UW -93 -33 -DS <br />On December 12, 19.94, the Board of County Commissioners awarded the <br />subject project in the amount of $683,372.00 to Tri -Sure Corporation <br />of Auburndale, Florida. (See attached.) <br />ANALYSIS <br />The project has been completed. All Indian River County <br />requirements have been met, and we have received DEP clearance. <br />Change Order No. 1 (attached) decreased the contract cost by <br />$2,015:30. The total cost of the project is therefore $681,356.70: <br />This decrease is due to field adjustments. The contractor has been <br />paid $556,419.60 to date. <br />The Department of Utility Services recommends approval of Change <br />Order No. 1, which reduces the contract cost by $2,015.30, and <br />approval of the final payment request for $124,937.10. <br />ON MOTION by Commissioner Eggert, SECONDED by <br />Commissioner Adams, the Board unanimously approved <br />Change Order No. 1 with Tri -Sure Corporation and <br />final payment request for $124,937.10, as <br />recommended by staff. <br />CHANGE ORDER #1 AND FINAL PAY REQUEST IS ON <br />FILE IN THE OFFICE OF THE CLERK TO THE BOARD <br />71 <br />DINE 279 1995 <br />
