NORTH U.S. #1 WATER MAIN - CHANGE ORDER NO. 5 <br />& FINAL PAY REQUEST - SPEEGLE CONSTRUCTION <br />The Board reviewed the following memo dated 7/17/95: <br />DATE:- JULY 17, 1995 /61 <br />TO: JAMES E. Mc <br />AND STAFFED CAPITAL PROJE S GINEER <br />BY: DEPARTMENT OF UTILITY SERVICES <br />SUBJECT: NORTH U.S. WATER MAIN <br />CHANGE ORDER NO. 5, PAY REQUEST NO. 8 <br />INDIAN RIVER COUNTY PROJECT NO. UW -93 -03 -DS <br />The portion of this project in Indian River County is complete, and the <br />Utilities Department wishes to finalize the contract paper work associated <br />with it. Please see attached Change Order No. 5 and Pay Request No. S. <br />ANALYSIS <br />The engineer, in conjunction with the Utilities Department, has put together <br />a --final quantity reconciliation change order in the amount of $41,905.85. <br />This amount represents a deduction to the portion of this project, which is <br />solely the responsibility of the County. We have also eliminated the <br />retainage on this portion of the contract as the lines have been tested, <br />cleared by DEP and accepted by the County. The attached pay request reflects <br />both the quantity adjustment, as well as the retainage -reduction. <br />RECOMMENDATION <br />The staff of the Department of Utility Services recommends the Board of <br />County Commissioners approve the attached Change Order No. 5 and Pay Request <br />No. 8, as presented. <br />ON MOTION by Commissioner Eggert, SECONDED by <br />Commissioner Adams, the Board unanimously approved <br />Change Order #5, and Pay Request #8, as set out in <br />staff's recommendation. <br />CHANGE ORDER & PAY REQUEST ON FILE IN OFFICE OF CLERK TO THE BOARD <br />36 <br />AUGUST 1, 1995 <br />
