This is an F.D.O.T. Small County Outreach Program (SCOP) funded project, FM No. 438074-1-54- <br />01. <br />Utilities <br />FP&L <br />NextEra Energy <br />Indian River County Traffic <br />Indian River County Utilities <br />Other Issues <br />• Review and comment with questions and clarifications as soon as possible, no further comments 10 <br />days prior to bid opening (Bid opening is November 10, 2021, deadline is November 1, 2021). <br />• The Notice to Proceed tentative issuance is January 3, 2022 <br />• Maintenance of Traffic: <br />o Contractor shall submit a Maintenance of Traffic Plan to the Traffic Division for approval. The <br />Maintenance of Traffic Plan shall meet the FDOT Index 600, FY 17-18 design standards. Any <br />deviations of the FDOT Index requires a Maintenance of Traffic Plan that is signed and sealed <br />by a Florida P.E. <br />o Access for all businesses and residences shall be maintained at all times. <br />• Sidewalk construction - true 6" thick concrete, 2% max. slope (no tolerance). <br />• Testing by TBD <br />• Work hours are 7:00 AM -5:00 PM, Monday/ -Friday <br />• A hurricane contingency plan must be provided prior to construction in case of a significant <br />tropical/weather event. <br />• Material storage and staging areas. <br />Project Requirements: <br />• Invoicing: Contractor shall submit monthly invoices — one (1) original Contractor's Application for <br />Payment (Section 00622 in Contract Docs), including Surety's Consent and all applicable releases of <br />liens. Project will run through E -Builder project management software. <br />• Contractors must meet deadlines in order to be in compliance. All deadlines are strictly enforced or <br />reimbursements cannot be made. <br />• Summary of required contractor form submittals checklist (Section 200-8 in contract documents). <br />• Bidder must be registered with and use, at their sole expense, the Department of Homeland <br />Security's E -Verify system (www.e-verify.gov) to confirm the employment eligibility of all newly <br />hired employees, as required by Section 448.095, F.S.. Owner, Contractor, and Subcontractors may <br />not enter into a contract unless each party to the contract registers with and uses the E -Verify <br />system. Contractor is responsible for obtaining proof of E -Verify registration for all Subcontractors. <br />This requirement applies to any provider of services or goods. <br />• Funding for this project is through a State grant. This is a County project but any proposed changes to <br />the project are required to be reviewed by F.D.O.T. for concurrence. The grant is of a reimbursement <br />type which means the County pays the Contractor and the State reimburses the County. F.D.O.T. <br />provides oversight to the project and in addition to other items requiring concurrence by F.D.O.T. are <br />contractual changes, time extensions and weather days.
