(See attached minutes.) On June 27, 1995, the Board of County <br />Commissioners approved Work Authorization No. 32, Project No. UW - <br />95 -17 -DS, to construct the water line replacement project with <br />Driveways, Inc., in the amount of $65,422.06. The contractor has <br />completed the project and has submitted his request for final <br />payment, which includes an increase of $1,023-40 as reflected on <br />Change Order No. 1 (See attached.) <br />The labor cost increase is due to the field location of an <br />additional 162 LF of 2 -inch galvanized water line replacement. The <br />following are the items that were added to accommodate the services <br />on this additional line: <br />1. 162 LF of additional 2 -inch PVC water line <br />2 • 226 LF of 1 -inch long services <br />3. 27 fittings <br />4. 7 tapping sleeves and valves <br />5.. replace 2 meter boxes and tie-in <br />6. replace all 75 old meters within the subdivision with new <br />meters <br />7. 1 blowoff <br />8. 56 LF of gravel drives <br />AAALYSIS <br />All Indian River County requirements have been met, and our testing <br />has been satisfactorily completed. The original labor contract <br />Price was $65,422.06, and the final labor contract price is <br />$66,445.46, which includes Change Order No. 1. The contractor has <br />previously been paid $50,948.61. This leaves a balance of <br />$15,496.85. <br />RECOMMEADATIOA <br />The staff of the Department of Utility Services recommends that the <br />Board of County Commissioners approve the attached Change Order No. <br />1 and final pay request of $15,496.85 to Driveways, Inc., as <br />payment in full for services rendered. <br />ON MOTION by Commissioner Adams, SECONDED by <br />Commissioner Bird, the Board by a 4-0 vote <br />(Commissioner Eggert being absent) approved Change <br />Order #1 and final pay request of Driveways, Inc. in <br />the amount of $15,496.85. <br />CHANGE ORDER #1 IS ON FILE IN THE OFFICE OF THE CLERK TO THE BOARD <br />CITY OF SEBASTIAN WASTEWATER TREATMENT PLANT <br />CONNECTION TO COUNTY UTILITIES <br />The Board reviewed the following memo dated 9/21/95: <br />r <br />27 g00K 96 F'A f 276 <br />OCTOBER 3, 1995 <br />
