Exhibit C - Page 1 <br />Page 1 of 3 <br />CHANGE ORDER BACK-UP DOCUMENTATION <br />FIN No.: 436379-1-54-01 & 436379-1-54-02 Contract No: IRC -1505 <br />Change Order No.: 02 Amount $ 841,817.35 and 60 Days <br />Title of Issue <br />Widening the 53rd Street Bridge from three (3) proposed travel lanes to five (5) proposed travel <br />lanes. <br />I. Entitlement Analysis <br />The purpose of this Change Order is to compensate the Contractor to construct the 53rd Street Bridge <br />over Lateral A Canal based on the redesigned 5 -lane typical section. Further, the Contractor agreed <br />to a Lump Sum (LS) amount and an addition of sixty (60) days Contract time. There are many benefits <br />to the redesign, including being able to accommodate future growth by being able to accommodate <br />additional travel lanes. Other benefits include upgrading the bridge structure to the current FDOT <br />Standards and changing from a prestressed/post tension single deck superstructure design to a <br />prestressed Florida Slab Beam (FSB) design which decreases long-term maintenance costs & <br />increases life of the structure. The net addition to the Contract will be $841,817.35, which includes <br />pay item overruns, jobsite overhead and other indirect costs associated with the 60 -day time <br />extension necessary to complete this work plus additional maintenance of traffic & erosion control for <br />the additional 60 -day period. <br />II. Time Analysis <br />The Contractor is pursuing a 60 -day time extension to complete the additional work. <br />III. Engineer's Estimate <br />a. Basis of Estimate <br />The Engineer's Estimate was based on the original Contract unit prices plus a "delta" based <br />on the current State -Wide -Average (SWA) minus the State -Wide -Average at time of original <br />bid, multiplied by the Engineer -of -Records' revised quantities for a 5 -lane design. Considering <br />the increase in unit cost is appropriate as the decision to proceed with the 5 -lane typical <br />section redesign occurred at this time and the Contractor was unable to proceed with <br />procurement of the original 3 -lane design components. To this we added Jobsite Overhead <br />& Other Indirect Expenses as a function of total Mobilization, Maintenance of Traffic (MOT) <br />and Erosion Control based on a per day rate using the original Contract time and original <br />Contract bid price multiplied by the 60 -day time extension. Mobilization, MOT & Erosion <br />Control are all original Contract pay item bid as one Lump Sum for original Contract time of <br />750 days.
