EXHIBIT A <br />Specialists follows up with clients regarding services and overall satisfaction. Through community <br />collaboration, IRCHSC has simplified navigating services and programs available in Indian River <br />County. One of these services being presumptive eligibility for pregnant women (PEPW) assists eligible <br />pregnant women to immediately receive prenatal care through Medicaid for up to 45 days, while their <br />eligibility for full Medicaid benefits is determined. The PEPW program is designed to improve pregnant <br />women's early access to outpatient prenatal services. The projected number of unduplicated children <br />to be served is three hundred ninety-six (396) and seven hundred four (704) adults. <br />5 - The Prevention Education and Community Enhancement (PEACE) program is designed to enhance <br />the Healthy Start system of care by streamlining initiatives that involve a primary focus on education <br />and outreach. PEACE will increase local capacity to address the mental, physical, and emotional needs <br />of pregnant mothers, partners, and families through direct education and outreach. IRCHSC identified <br />prioritized initiatives through community assessment and FIMR. PEACE education programs for <br />community members (e.g., mothers, families) and professionals (OBs, pediatricians, mental health <br />professionals) include perinatal mental health, perinatal bereavement, and empowerment and well- <br />being of the other parent, and birth and postpartum support. The projected number of unduplicated <br />adults to be served is five hundred fifty (550). <br />6 - Healthy Families Indian River County is a home visitation program. Services are initiated during <br />pregnancy and/or if the infant is 3 months of age or younger, for mothers who are vulnerable for meeting <br />the challenges of adding a new member to the family. Some mothers have diagnosed mental health <br />issues and need positive parenting techniques to help raise their baby. Participants also learn about <br />the importance of healthcare, well -baby checks, childhood immunizations, and family planning. Also, <br />Family Support Workers (FSW's) conducts regular developmental screenings of the children along with <br />the families to identify if infants and young children need further evaluation for developmental delays <br />that affect readiness for school. FSW's help families access quality daycare and services in the <br />community. Referrals to community partners assist in aiding families to gain assistance in other areas <br />of need. The projected number of unduplicated children to be served is two hundred twenty (220) and <br />one hundred thirty (130) adults. <br />
