()-� -1:S 1 <br />MEMORANDUM OF AGREEMENT <br />BETWEEN <br />INDIAN RIVER COUNTY HEALTHY START COALITION, INC. <br />AND <br />INDIAN RIVER COUNTY BOARD OF COUNTY COMMISSION <br />I. Purpose: <br />The purpose of this Memorandum of Agreement (MOA) between Indian River County Healthy <br />Start Coalition ("Coalition"), and the Indian River County Board of County Commission <br />("Provider"), is to provide for the coordination and implementation of Healthy Start Parent <br />Education Services in Indian River County (the "County"). The coordination and <br />implementation of Healthy Start Parent Education Services in the County are to be consistent <br />with the Service Delivery Plan developed by the Coalition. The Coalition has developed its <br />Service Delivery Plan in order to provide for a comprehensive prenatal and infant health care <br />system for those pregnant women and infants most at risk for poor pregnancy, health, or <br />developmental outcomes in the County. The Coalition now desires to designate Indian River <br />County Board of County Commission, as the "Provider", the entity responsible for the <br />coordination and implementation of the action steps with regard to Parent Education Services as <br />delegated in the Service Delivery Plan during the term of this Agreement, and the Provider <br />desires to accept such designation. <br />II. Program Description <br />The Indian River County Board of County Commission hereinafter referred to, as "Provider" will <br />provide services outlines below in Section VI to Indian River County parents, targeting parents of <br />toddlers of age one to three years old. <br />III. Eligibility <br />The Provider will serve all families who have a pregnant woman or child up to age three years in <br />Indian River County who attend and participate in Parent Education. Families may be identified <br />through the Healthy Start prenatal or infant screening as scoring at risk, may be self-referred or <br />referred by others based on factors other than score. <br />IV. Term <br />The term of this MOA shall be deemed to have commenced upon signature of this MOA and will <br />continue through and including June 30, 1999, or until 30 days notice of termination by the Provider <br />or the Coalition. This funding will continue as funds are made available. <br />V. Compensation and Budget <br />In consideration of the services provided by the Provider, the Coalition will pay the Provider a sum <br />not to exceed $ 24,137.00 per fiscal year. Payments shall be made to reimburse costs upon <br />presentation of invoice and report by the Provider, submitted to the Coalition. Payment will be made <br />within thirty days after presentation of invoice and report to the Coalition. <br />A two-month advance of $ 4,022.83 will be advanced to the Provider to cover start-up costs. This <br />advance will be deducted from the final invoice. <br />These funds will be used to support services and staff as delineated in the budget in the Attachment. <br />
