BUTLER, CLERK <br />STATE OF FLORIDA <br />DEPARTMENT OF ENVIRONMENTAL PROTECTION <br />GRANT WORK PLAN <br />AGREEMENT NO.22SRP09 <br />ATTACHMENT 3 <br />PROJECT TITLE: South Oslo Riverfront Conservation Area Floodplain Restoration <br />PROJECT LOCATION: The Project is located within Indian River County, Florida; Lat/Long (27.58298, <br />-80.36500). <br />PROJECT DESCRIPTION: <br />The St. Johns River Water Management District (Grantee) will conduct the South Oslo Riverfront <br />Conservation Area Floodplain Restoration Project (Project) to remove an existing 1,100' perimeter dike to <br />improve exchange between impounded mangroves and the Indian River Lagoon (IRL). The Project will <br />restore the area to a more natural hydroperiod in the impoundment that will benefit wildlife that use the <br />mangroves, coastal hammock, and surrounding fisheries habitat. The Project includes design, permitting, <br />and construction. <br />TASKS AND DELIVERABLES: <br />Task 1: Design and Permittin <br />Description: The Grantee will acquire professional services for the engineering and design of the removal <br />of an existing 1,100' perimeter dike and the restoration of the land similar to adjacent habitat and shoreline <br />vegetation. The Grantee will obtain all necessary permits for construction of the Project. Design and <br />permitting activities may include coastal or civil engineering analyses, preparation of plans and <br />specifications, physical and environmental surveys, cultural resource surveys, design -level geotechnical <br />services, environmental analyses, orthophotography, plan formulations and other necessary studies for <br />obtaining environmental permits, and other Project -related authorizations. <br />Deliverables: The Grantee's will submit all final design documents as signed by a Florida -registered <br />Professional Engineer. The Grantee will also submit final permit documents from all appropriate state and <br />federal regulatory agencies. <br />Task 2: Construction <br />Description: The Grantee will remove an existing 1,100' perimeter dike and restore the land similar to the <br />adjacent habitat and shoreline vegetation. The Grantee will complete all Construction activities in <br />accordance with the construction contract documents. Project costs associated with the Construction task <br />include work approved through construction bids and/or construction -phase engineering and monitoring <br />services contracts. Eligible activities may include mobilization, demobilization, construction observation <br />or inspection services, physical and environmental surveys, and mitigation projects. Construction shall be <br />conducted in accordance with all state or federal permits. <br />Deliverables: The Grantee will submit: 1) a copy of the final design and record (as -built) drawings; 2) a <br />signed acceptance of the completed work to date, as provided in the Grantee's Certification of Payment <br />Request; 3) a signed Engineer's Certification of Payment Request; and 4) when construction is complete, a <br />Certificate of Occupancy (if applicable) and a Certificate of Completion signed by a Florida -registered <br />Professional Engineer. <br />DEP Agreement No.: 22SRP09 <br />Page 1 of 2 <br />Rev. 2.11.22 <br />
