A TRUE COPY <br />CERTIFICATION ON LAST PAGE <br />RYAN L. BUTLER, CLERK <br />of State, Division of Historical Resources, Review and Compliance Section at (850) 245-6333. <br />Project activities shall not resume without verbal and written authorization. In the event that <br />unmarked human remains are encountered, during permitted activities, all work shall stop <br />immediately and the proper authorities notified in accordance with Florida Statutes, Section <br />872.05. Source of condition: National Historic Preservation Act (NHPA), Monitoring Required: <br />No <br />If human remains or intact archaeological deposits are uncovered, work in the vicinity of the <br />discovery will stop immediately and all reasonable measures to avoid or minimize harm to the finds <br />will be taken. The Sub -Recipient will ensure that archaeological discoveries are secured in place, <br />that access to the sensitive area is restricted, and that all reasonable measures are taken to avoid <br />further disturbance of the discoveries. The Sub -Recipient contractor will provide immediate notice <br />of such discoveries to the Sub -Recipient. The Sub -Recipient shall contact the Florida Division of <br />Historic Resources and FEMA within 24 hours of the discovery. Work in the vicinity of the discovery <br />may not resume until FEMA has completed consultation with SHPO, Tribes, and other consulting <br />parties as necessary. In the event that unmarked human remains are encountered during <br />permitted activities, all work shall stop immediately and the proper authorities notified in <br />accordance with Florida Statutes, Section 872.05. Source of condition: National Historic <br />Preservation Act (NHPA), Monitoring Required: No <br />The subrecipient shall ensure that all debris staging sites are pre -authorized by FDEP. The <br />subrecipient shall ensure that all debris is separated and disposed of in a manner consistent with <br />FDEP solid waste facility disposal at permitted facilities guidelines or at a disposal site or landfill <br />authorized by FDEP. The subrecipient is responsible for ensuring contracted staging and disposal <br />of debris also follows these guidelines. Failure to comply with these conditions may jeopardize <br />FEMA funding; verification of compliance will be required at project closeout. Source of <br />condition: Resource Conservation and Recovery Act, aka Solid Waste Disposal Act (RCRA), <br />Monitoring Required: No <br />k. The project sponsor must obtain and comply with all applicable permit and approvals required by <br />Federal, State, Tribal, and local regulatory agencies. FEMA would consult with the State/Tribal Historic Preservation Office (SHPO/THPO) on project <br />specific activities for any project that has the potential to affect previously undisturbed areas or <br />historic properties. If during the course of any ground disturbance related to this project, cultural <br />materials are inadvertently discovered, the project would be immediately stopped and the <br />SHPO/THPO and FEMA notified. FEMA would consult with the U.S. Fish and Wildlife Service on a project specific basis for any <br />actions that have the potential to affect biological resources, including threatened and endangered <br />species. Perform skidding or yarding operations when soil conditions are such that soil compaction, <br />displacement, and erosion would be minimized. Use low ground pressure equipment when practicable, particularly on equipment traveling over <br />large portions of units with sensitive soils or site conditions.
